针灸联合调周法治疗脾虚痰湿型多囊卵巢综合征不孕的疗效观察

Observations on the Efficacy of Acupuncture and Moxibustion plus Menstrual Cycle-regulating Method for Infertility in Patients with Polycystic Ovary Syndrome of Spleen Deficiency and Phlegm-dampness Type

目的观察针灸联合调周法治疗脾虚痰湿型多囊卵巢综合征(PCOS)不孕临床疗效。方法 126例脾虚痰湿型PCOS患者,随机分为观察组和对照组,每组63例。对照组采用调周法治疗,观察组在对照组基础上采用针灸治疗。观察两组治疗前后患者体质量、体质量指数(BMI)、血清性激素与胰岛功能相关指标变化,并比较两组的临床疗效、周期排卵率及临床妊娠率。结果观察组总有效率为84.1%,高于对照组的71.4%,差异有统计学意义(P<0.05)。两组治疗后血清总睾酮(T)、促黄体生成素(LH)含量较治疗前降低,观察组体质量、BMI较治疗前和对照组降低,差异有统计学意义(P<0.05)。两组治疗后胰岛素敏感指数(ISI)较治疗前上升,空腹胰岛素(INS)含量、胰岛?细胞功能指数(HBCI)较治疗前下降,观察组INS含量低于对照组,差异有统计学意义(P<0.05)。观察组周期排卵率高于对照组,差异有统计学意义(P<0.05)。结论针灸联合调周法治疗可改善PCOS患者胰岛素抵抗情况,降低体质量指数,调控内分泌稳态,促进抗生殖激素和生殖激素的平衡,增大周期排卵率,提高妊娠率。

Objective To investigate the clinical efficacy of cupuncture and moxibustion plus menstrual cycleregulating method a in treating infertility in patients with polycystic ovary syndrome(PCOS)of spleen deficiency and phlegm-dampness type.Method One hundred and twenty-six patients with PCOS of spleen deficiency and phlegm-dampness type were randomized to observation and control groups,63 cases each.The control group received menstrual cycle-regulating method and the observation group,acupuncture and moxibustion in addition.Body weight,body weight index(BWI),serum sex hormones and islet function-related indicators were measured in the two groups before and after treatment.The clinical therapeutic effects,ovulation cycle rates and clinical pregnancy rates were compared between the two groups.Result The total efficacy rate was 84.1%in the observation group,which was higher than 71.4%in the control group,with a statistically significant difference(P<0.05).After treatment,serum total testosterone(T)and luteinizing hormone(LH)contents decreased in the two groups compared with before,and body weight and BWI decreased in the observation group compared with before and in the control group,with statistically significant differences(P<0.05).After treatment,insulin sensitivity index(ISI)increased,and fasting insulin(INS)and Homa?-cell function index(HBCI)decreased in the two groups compared with before,and INS content was lower in the observation group than in the control group,with statistically significant differences(P<0.05).The ovulation cycle rate was higher in the observation group than in the control group,with a statistically significant difference(P<0.05).Conclusion Acupuncture and moxibustion plus menstrual cycle-regulating method can reduce insulin resistance,decrease body weight index,regulate endocrine homeostasis,promote the balance of anti-reproductive hormones and reproductive hormones,increase ovulation cycle rate and raise the pregnancy rate in PCOS patients.
