单胎妊娠初产妇产程进展的相关因素分析及分娩前运动疗法干预效果

Analysis of Related Factors of Labor Progress of Singleton Pregnancy Primipara and Intervention Effect of Pre-partum Exercise Therapy

目的:探讨单胎妊娠初产妇产程进展的相关因素及分娩前运动疗法干预效果。方法:选取2018年3月~2019年3月某院妇产科90例妊娠产妇,按入院顺序分为对照组和观察组各45例,对照组常规分娩,观察组运动疗法干预。收集产妇基本资料,记录其产程时间、分娩结局及新生儿Apgar评分。结果:单因素结果显示,孕周、胎儿体重、胎膜早破、使用催产素、分娩镇痛及运动疗法对产程进展有影响(P<0.05);经Logistic回归分析,胎儿体重、胎膜早破、使用催产素及运动疗法是影响妊娠产妇产程进展的因素(P<0.05);观察组各产程及总产程时间短于对照组(P<0.05);观察组自然分娩及阴道助产人数多于对照组(P<0.05);观察组新生儿Apgar评分高于对照组(P<0.05)。结论:胎儿体重、胎膜早破、使用催产素及运动疗法是单胎妊娠初产妇产程进展的影响因素,经运动疗法干预后,产妇分娩结局更优,新生儿生存质量更高。

Objective:To investigate related factors of labor progress of singleton pregnancy primipara and intervention effect of pre-partum exercise therapy.Methods:90 singleton pregnancy primipara in a hospital from March 2018 to March 2019 were selected,and divided into control group and observation group according to the order of admission,45 cases in each group.The control group was treated with routine delivery,the observation group was treated with exercise therapy intervention.The data of parturient women were collected,and the duration of labor,outcome and Apgar score were recorded.Results:Univariate results showed that gestational age,fetal weight,premature rupture of membranes,oxytocin,labor analgesia and exercise therapy had an effect on the labor progress(P<0.05).Logistic regression analysis showed that fetal weight,premature rupture of membranes,use of oxytocin and exercise therapy were the factors affecting the progress of labor in pregnant women(P<0.05).The duration of every stage of labor and the total labor in the observation group was shorter than that in the control group(P<0.05).The number of natural delivery and vaginal midwifery in the observation group was higher than that in the control group(P<0.05).Apgar score was higher in the observation group than in the control group(P<0.05).Conclusion:Fetal weight,premature rupture of membranes,use of oxytocin and exercise therapy are the factors that affect the progress of labor in the first trimester of a single pregnancy.After the intervention of exercise therapy,maternal delivery outcome is better,neonatal quality of life is higher.
