摘要 目的:评价磁共振(MR)三维连续动脉自旋标记(3D-ASL)联合DWI成像对缺血性脑卒中的临床价值。方法:选取我院40例经临床诊断的急性缺血性脑卒中的患者,均行DWI及3D-ASL检查,测量梗死区ADC值、ASL灌注异常区及两者不匹配区(即IP区)脑血流量(CBF),并分别与对侧比较。结果:40例患者DWI序列均可见梗死区异常高信号,19例存在理论IP,2例梗死区无灌注异常改变,2例梗死区呈高灌注改变。梗死区的ADC值、CBF值及IP区CBF值明显低于对侧(P<0.01);梗死区CBF值低于IP区(P<0.01)。结论:3D-ASL联合DWI成像技术可以用来评估缺血半暗带、识别供血动脉的闭塞、发现梗塞周围的高灌注,并评估侧支循环,为临床早期诊断及治疗方案的选择提供依据。 Objective To evaluate the application value of 3D-ASL combined with DWI technique of MRI in cerebral ischemic.Methods 3D-ASL and DWI were performed in 40 cases of acute ischemic cerebral infaction in our hospital.We respectively measured the value of ADC and the cerebral blood flow(CBF)in the lesions and the theoretical ischemic penumbra(IP),and compared with contra.Results All of the 40 cases by DWI detected obvious abnormalities,and 19 cases had theoretical IP.2 cases had no abnormal perfusion and 2 cases had hyperperfusion in the infarct area.ADC value and the CBF of infarct area and the CBF of IP were significantly lower than contra(P<0.01).The CBF of infarction area was lower than IP(P<0.01).Conclution 3D-ASL combined with DWI technology of MRI can be used to evaluate ischemic penumbra,identify the artery occlusion,detect hyperperfusion around infarction and evaluate collateral circulation,Which is beneficial to early diagnosis and the selection of best treatment project.
