婴幼儿血管瘤消退后远期纤维脂肪残余及皮肤质地改变 被引量:4

Long term changes of fiber fat residue and skin texture after the disappearance of infantile hemangioma

摘要 目的研究婴幼儿血管瘤消退后远期皮肤改变的情况,探讨不同皮肤后遗症的治疗方案。方法对2008年1月至2011年12月来我科就诊的671例深部型及混合型婴幼儿血管瘤患儿进行随访,统计总结血管瘤消退后的皮肤情况。结果最终收取280例有随访照片的患儿,消退后的正常皮肤率为39.6%。在瘤体消退后皮肤残留外观改变的事件中,毛细血管扩张的发生率最高(34.3%),纤维脂肪残余次之(23.6%),皮肤皱褶及疤痕发生率较低(8.6%及10%)。卡方检验证实,混合型瘤体消退后正常皮肤率(30.7%)显著低于深部型(61.7%)。结论局部隆起的婴幼儿血管瘤,消退后完全恢复为正常皮肤的概率较低,可对患儿造成永久性的外观改变。瘤体的类型可能影响最终消退后的皮肤外观,但相关因素的分析需要进一步的数据证实。 Objective To investigate the long term changes of fiber fat residue and skin texture after infantile hemangioma regression and to explore the treatment of different skin sequelae.Methods We followed up 671 patients with mixed or deep infantile hemangioma presented to our department during period between January 2008 and December 2011.Statistical summary of the skin condi tion after the regression of hemangioma was performed.Results Finally,280 children with follow up photos were enrolled.The normal skin rate after involution was 39.6%.The incidence of telangiectasia was the highest(34.3%)followed by fibrofatty residue(23.6%)and anetodermic skin and scars(8.6%and 10%,respectively).Pearson chi square test confirmed that the normal skin rate of mixed in fantile hemangiomas(30.7%)was significantly lower than that of deep infantile hemangiomas(61.7%).Conclusion Mixed and deep infantile hemangiomas have a low complete regression rate and can often cause permanent changes in skin texture or appearance.Lesion type may be a risk factor for regression outcome.However,the analysis of related factors needs further data confirmation.
