哮喘急性发作期血清IL-21、IL-6、IL-17水平的变化及意义 被引量:7

摘要 目的分析哮喘急性发作期血清白细胞介素 21(IL 21)、IL 6、IL 17水平的变化及意义。方法通过酶联免疫吸附法测定急性发作期哮喘患儿60例(急性发作组)、缓解期哮喘患儿54例(缓解组)和体检健康儿童50例(对照组)血清IL 21、IL 5、IL 17水平。比较不同严重程度急性发作期哮喘患儿血清IL 21、IL 5、IL 17水平;分析血清指标间的关系,对急性发作期哮喘血清IL 21、IL 6、IL 17行ROC曲线分析。结果急性发作组血清IL 21、IL 6、IL 17水平高于缓解组和对照组(P<0.05);血清IL 21、IL 6、IL 17水平随急性发作期哮喘病情加重而上升(P<0.05);与治疗前比较,急性发作期治疗后血清IL 21、IL 6、IL 17水平下降(P<0.05);血清IL 21与IL 17、IL 6均正相关(P<0.05);血清IL 21、IL 6、IL 17水平对急性发作期哮喘诊断曲线下面积分别为0.966、0.948、0.987。结论血清IL 21、IL 6、IL 17协同参与急性发作期哮喘发生,曲线下面积均超过0.9,诊断价值高,可作为哮喘急性发作诊断及病情监测重要指标。 Objective To analyze the changes and significance of serum levels of interleukin 21(IL 21),IL 6 and IL 17 in acute asthmatic attacks.Methods Enzyme linked immunosorbent assay(ELISA)was used to detect serum levels of IL 21,IL 5,and IL 17 from 60 children with acute exacerbation(acute attack group),54 children with remission asthma(remission group)and healthy children with physical examination.Serum levels of IL 21,IL 5 and IL 17 levels were compared in children with acute asthma at differ ent levels of severity.Pearson correlation analysis was used to analyze the relationship between serum indicators,and serum IL 21,IL 6 and IL 17 in acute attack by using ROC curve analysis.Results he levels of serum IL 21,IL 6 and IL 17 in the acute attack group were significantly higher than those in the remission group and the control group(P<0.05).Serum levels of IL 21,IL 6 and IL 17 were increased with the exacerbation of acute asthma(P<0.05).Compared with before treatment,the serum levels of IL 21,IL 6 and IL 17 were significantly decreased in children with acute asthma(P<0.05).Serum IL 21 was significantly associated with IL 17 and IL 6(P<0.05).The area under the asthma diagnosis curve of the levels of serum IL 21,IL 6 and IL 17 were 0.966,0.948 and 0.987,respectively.Conclusion Serum IL 21,IL 6 and IL 17 are involved in the acute exacerbation of asthma.The area under the curve is more than 0.9 and the diagnostic value is high.It can be used as an important indicator for the diagnosis and monitoring of acute asthma.
