赋能教育模式在髋关节置换后功能锻炼中的应用 被引量:5

Application of empowerment theory in functional exercise following hip arthroplasty

摘要 目的探讨赋能教育模式在髋关节置换患者功能锻炼中的应用效果。方法选取2016年3月至2018年3月广州市红十字会医院骨科收治的118例髋关节置换患者。纳入标准为首次行髋关节置换术的患者,意识清楚,能用语言和文字进行有效交流,知情同意。排除合并严重心、肺、肾等重要脏器疾病或恶性肿瘤患者,有精神疾病病史或长期服用精神科药物者。将118例行髋关节置换术患者随机分成观察组(60例)和对照组(58例)。观察组实施赋能教育模式进行功能锻炼指导,对照组采用常规方法进行功能锻炼指导。采用独立样本t检验比较两组患者的功能锻炼自我效能得分、髋关节功能得分,采用卡方检验比较两组患者的功能锻炼依从性。结果观察组患者功能锻炼自我效能得分(88±15)高于对照组(67±14),差异有统计学意义(t=7.891,P<0.01);观察组患者功能锻炼依从率(75%)高于对照组(52%),差异有统计学意义(χ^2=7.229,P<0.05);观察组患者髋关节功能得分(80±7)高于对照组(75±7),差异有统计学意义(t=3.918,P<0.01)。结论赋能教育模式能够有效提高髋关节置换患者功能锻炼自我效能及锻炼依从性,促进髋关节功能康复。 Objective To explore the application effect of empowerment theory in functional exercise of hip arthroplasty patients.Methods A total of 118 hip arthroplasty patients admitted to the Department of Orthopaedics,Guangzhou Red Cross Hospital from March 2016 to March 2018 were selected.The inclusion criteria were patients with first-time hip arthroplasty,with clear consciousness,effective communication and informed consent in language and writing,and informed consent.The patients with lung,kidney and other important organ diseases or malignant tumors,and those with a history of mental illness or long-term use of psychotropic drugs were excluded.The arthroplasty patients were randomly divided into the observation group(60 cases)and the control group(58 cases).The patients in observation group were treated with empowerment education in functional exercise while the control group received usual nursing care.The independent sample t test was used to compare the self-efficacy score of functional exercise and hip function score between the two groups of patients.Chi-square test was used to compare functional exercise compliance between the two groups.Results The self-efficacy scores of functional exercise in the observation group(88±15)were higher than those in the control group(67±14),and the difference was statistically significant(t=7.891,P<0.01).The compliance rate of functional exercise in the observation group(75%)was higher than that in the control group(52%),and the difference was statistically significant(χ^2=7.229,P=0.027).The hip function scores of the observation group(80±7)were higher than those of the control group(75±7),and the difference was statistically significant(t=3.918,P<0.01).Conclusion The empowerment theory can effectively improve the functional self-efficacy and functional exercise compliance of hip arthroplasty patients and promote the rehabilitation of hip function.
