超声引导下康复新液穿刺冲洗和切开引流在乳腺脓肿治疗中的应用

Value of ultrasound-guided puncture, irrigation and drainage using Kangfuxin fluid for patients with breast abscess

目的:探讨超声引导下康复新液穿刺冲洗和切开引流在乳腺脓肿治疗中的应用价值,以更好地治疗乳腺脓肿。方法:选取2017年1月至2019年1月某院乳腺脓肿患者100例,依据随机数字表分为切开组和指导组,每组50例。切开组给予传统康复新液冲洗和切开引流治疗,指导组超声引导下通过动态观察乳腺脓肿图像进行康复新液穿刺冲洗和切开引流治疗,比较2组手术情况、术后康复情况、疼痛视觉模拟评分(visual anglogue scale,VAS)、并发症率及乳房外观满意情况。采用SPSS 22.0统计学软件进行数据分析。结果:指导组手术时间、切口长度和术后脓腔愈合时间、换药次数、瘢痕长度、住院时间、并发症率明显低于切开组,差异有统计学意义(P<0.05);切开组和指导组治疗3、7 d后VAS评分明显低于治疗前,指导组治疗3、7 d后VAS评分明显低于切开组,差异均有统计学意义(P<0.05);指导组乳房外观满意率明显高于切开组,差异有统计学意义(P<0.05)。结论:超声引导下进行康复新液穿刺冲洗和切开引流治疗乳腺脓肿可有效减轻患者的手术创伤、减少并发症,有利于改善患者预后,值得临床推广。

Objective To discuss the value of ultrasound-guided puncture,irrigation and drainage using Kangfuxin fluid in the patients with breast abscess in order to better treat the breast abscess.Methods Totally 100 patients with breast abscess in some hospital from January 2017 to January 2019 were selected,and divided into an incision group and a guidance group according to random number table,50 cases in each group.The incision group was treated with traditional puncture,irrigation and drainage using Kangfuxin fluid,and the guidance group was treated with ultrasound-guided puncture,irrigation and drainage using Kangfuxin fluid.The two groups were compared over surgery,postoperative rehabilitation,visual anglogue scale(VAS),complications and breast appearance.SPSS 22.0 software was used for data analysis.Results The guidance group behaved better significantly then the incision group in operation time,incision length,healing time of pus cavity,dressing change times,scar length,hospitalization time and complication rate(P<0.05);VASs 3 and 7 days after operation in the two groups were lower than those before operation,and the guidance group had the VAS lower significantly than those of the incision group(P<0.05);the guidance group gained advantages over the incision group in breast appearance,and the difference was statistically significant(P<0.05).Conclusion Ultrasound-guided puncture,irrigation and drainage using Kangfuxin fluid can effectively reduce surgical trauma and complications,and it is helpful to improve the prognosis of the patients,which is worthy promoting clinically.
