“干”是保证住院医师规范化培训质量的关键

“Practice” is the key to quality of standardized residency training

住院医师规范化培训(住培)的目标是把医学生培养成为独立行医的医师,即在规定时间内,通过大量临床实践让医学生完成向独立行医医师的转变。住院医师从参加培训的第一天起,就已经从医学生转变为医师,只不过是在带教医师监督下行医的医师。培训住院医师与培养医学生完全不同,其培训方式、教学方法、考核标准、带教老师的职责、管理方式都不能沿用医学生的教学方法。住培必须放手让住院医师去临床实际工作,为此有必要控制基地招生数量,保证住院医师的工作负荷,改善住培教学组织结构,在放手让住院医师临床实践的同时保障患者安全,并通过合理有效的基地评估和检查督促这个目标的实现。

Residency is an essential dimension for the transformation from medical students to independent practitioners. Therefore, clinical practice is important for residency training. From the day one of residency training, residents have already become medical doctors, who will practice medicine under supervisions. The residency training should not be the same as medical student education;therefore, it should not cope the strategies and methods for medical student education to train residents. All the designs for residency training must meet the needs for residents to practice. For this reason, it is necessary to provide a reasonable working load for each trainee by limiting resident numbers in the program. Also, it is essential to construct well training structures and organize training activities, which can let residents practice medicine and ensure patient safety at the same time. It is also necessary to perform effective accreditations for each residency program to promote the best results for training qualified doctors nationwide.
