以岗位胜任力为核心的口腔全科住院医师规范化培训的实践探索

2002年ACGME提出住院医师"6大核心能力"概念,分别为:患者照顾、医学知识、专业素养、以系统为基础的实践、以实践为基础的学习和提高、人际关系与沟通技巧。本基地以此为根本,结合口腔全科专业特点,强调岗位胜任力是住院医师的基本能力,逐步建立健全了"以岗位胜任力为核心的口腔全科住院医师规范化培训"体系,并进行了积极的实践与探索。执行标准化门诊工作流程,分为6个环节:接诊、查体、计划、处置、宣教、随访;CBL教学模式串联住培教学活动,包括教学查房、疑难问题讨论、小讲课、病例汇报;完善实施住院医师分级考核制度,月度考核、出科考核、年度考核、结业考核,逐级进阶;加强科室文化建设、人文教育、入科教育,夯实基础,丰富的平台资源和人文活动有效地促进了住院医师临床、教学、科研能力循序渐进地稳步提高,从而全面提升基地的培训质量,培养和锻炼高效、优质的教学团队,为口腔全科专业教学积累经验。

In 2002, ACGME identified 6 core competencies to evaluate their residents in training, which are: Patient Care, Medical Knowledge, Practice Based Learning and Improvement, Interpersonal and Communication Skills, Professionalism, and Systems Based Practice. Based on these basic principles and the characteristics of oral medicine, the professional training base of stomatology in Jiangsu Province Hospital proposed that the clinical competency is the basic abilities of residents. The competency-orientated system of standardized residency training for stomatology is gradually established through practice and exploration energetically. The standardized outpatient treatment procedure contained 6 steps: Reception, Examination, Planning, Management, Education and Fllow up. Casebased learning is applied to improve the medical education, including teaching ward inspections, difficult cases discussion, lectures and case reports. The evaluation system, as a significance parameter to assess the results of training, is divided into monthly assessment, department assessment, annual assessment and graduation assessment. Enhancing cultural construction and humanities education is also the indispensable part of residency training system, which effectively promoted the capabilities of residents’ clinical skills, teaching abilities and scientific research abilities and was of great significance to comprehensively improve the quality of standardized residency training bases of stomatology.
