摘要
目的:通过建立慢性阻塞性肺疾病(COPD)肺气肿大鼠模型,观察补肺汤对慢性阻塞性肺疾病肺气肿模型大鼠肺组织病理学及血清中IFN-γ、IL-4表达水平的影响,探讨补肺汤的治疗作用及可能的作用机制。方法:将40只清洁级健康SD大鼠,随机分为四组,每组各10只。空白对照组、模型组、西药对照组(脾氨肽组)、中药治疗组(补肺汤组)。除空白对照组以外其余各组均予以气道内注射脂多糖联合烟熏的方法制作COPD肺气肿动物模型,空白对照组与此同时在气道内注射等量生理盐水,空白对照组不予以烟熏。造模结束后,分别给予生理盐水及相应药物灌胃,2次/d,连续给药14 d。第15天开始将大鼠处死,取肺组织观察其病理学变化,用ELISA法检测血清中IFN-γ、IL-4炎性因子的水平。结果:与模型组相比,补肺汤可减轻肺组织的病理损害。与空白对照组相比,模型对照组大鼠血清中IFN-γ、IL-4的含量均有显著差异;与模型对照组相比,西药对照组与中药治疗组大鼠血清中IFN-γ、IL-4的含量有显著差异;与西药对照组相比,中药治疗组血清中IFN-γ、IL-4的含量无显著差异。结论:补肺汤对COPD肺气肿有良好的治疗作用,其作用机制可能是通过调控血清中IFN-γ、IL-4的表达,从而调节了Th1/Th2细胞因子平衡有关。
Objective:To observe the effect of Bufei decoction on lung histopathology and serum IFN-γand IL-4 expression in rats with chronic obstructive pulmonary disease(COPD)and emphysema,and to explore the therapeutic effect and possible mechanism of Bufei decoction on COPD emphysema.Methods:40 SD healthy rats were randomly divided into 4 groups with 10 rats in each group.Blank control group,model control group,western medicine control group,traditional Chinese medicine treatment group.In addition to the blank control group,all the other groups were given intra-airway injection of lipopolysaccharide combined with smoking method to make animal models of COPD emphysema.At the same time,the blank control group was injected with the same amount of saline in the airway,and no passive smoking was given.After the successful establishment of the model,the drug was given by intragastric administration the next day.The blank control group and model group were given normal saline,the western medicine group was given splenic ammonia peptide oral freeze-dried powder,and the Chinese medicine treatment group was given Bufei decoction for 14 days.On the 15th day,the femoral artery of rats was cut open and bloodletting was executed.The pathological changes of the right lung tissue were observed.The levels of IFN-γand IL-4 inflammatory factors in serum were detected by ELISA.Results:In the blank control group,tracheal mucosa ciliated columnar epithelial cells were intact,respiratory tract and alveolar epithelial structure were intact,alveolar tissue was uniform,alveolar interstitial thickness was appropriate,macrophages were rarely seen in alveolar area,and there were almost no secretions and inflammatory cells in bronchial cavity.In model group,mucosal epithelium was incomplete,trachea and bronchial ciliary epithelium was partially exfoliated,goblet cells and glands were significantly proliferated and hypertrophic,bronchial mucosal folds were increased and lengthened,and protruded into the lumen;lymphocyte and macrophage-dominated inflammatory cells infiltrated the bronchial mucosa and submucosal layer at all levels,and terminal bronchioles were observed.The distal tracheal stenosis,respiratory bronchioles and alveolar ducts cystic dilatation,irregular enlargement of alveolar cavity,thinning,swelling,rupture or fusion of alveolar wall formed bullae.Compared with the model group,the lesions mentioned above were alleviated in the western medicine control group and the traditional Chinese medicine treatment group.Compared with the blank control group,the serum levels of IFN-γand IL-4 in the model control group were significantly different;compared with the model control group,the serum levels of IFN-γand IL-4 in the western medicine control group and the traditional Chinese medicine treatment group were significantly different;compared with the western medicine control group,the serum levels of IFN-γand IL-4 in the traditional Chinese medicine treatment group were significantly different.There are differences.Conclusion:Bufei decoction has a good therapeutic effect on COPD emphysema.Its mechanism may be through promoting the secretion of IFN-γin serum and inhibiting the secretion of IL-4,thus regulating the balance of Th1/Th2 cytokines.
