党黄抗癌汤配合肿瘤细胞减灭术中腹腔热灌注及化疗治疗Ⅳ期上皮性卵巢癌临床研究

Danghuang Kang’ai decoction combined with cytoreductive surgery and intraperitoneal hyperthermic perfusion in the treatment of stage IV ovarian cancer

目的:探讨党黄抗癌汤配合肿瘤细胞减灭术中腹腔热灌注及化疗治疗Ⅳ期上皮性卵巢癌的临床疗效。方法:将234例Ⅳ期上皮性卵巢癌患者随机分为观察组和对照组,每组各为117例。对照组给予肿瘤细胞减灭术中腹腔热灌注治疗,术后进行TP方案(多西他赛+顺铂)化疗,观察组在对照组基础上另给予党黄抗癌汤,1剂/次,1次/d,共治疗105 d。治疗前后,评价两组患者情感状况、附加关注、生理状况、社会/家庭状况及功能状况等生活质量评分水平,检测两组患者外周血中CD 3^+、CD 4^+、IgA、IgG和IgM水平,观察治疗期间各项毒副作用发生率。治疗结束后,对两组患者进行为期2年的随访,观察患者1年、2年的生存率,总生存时间(OS)及无疾病进展生存时间(PFS)。结果:治疗后,观察组情感状况、附加关注、生理状况、社会/家庭状况及功能状况评分分别为(18.35±1.78)、(26.78±2.94)、(34.28±3.35)、(56.21±5.67)及(68.34±6.78)分,改善程度均明显优于对照组(P<0.05),CD 3^+、CD 4^+、IgA、IgG和IgM含量分别为(61.11±6.56)%、(46.43±4.75)%、(2.85±0.32)g/L、(15.12±1.65)g/L及(2.39±0.25)g/L,均明显高于对照组(P<0.05)。远期疗效评价中,观察组1年、2年的生存率,OS及PFS均明显高于对照组(P<0.05)。治疗期间,观察组各项毒副作用发生率均明显低于对照组(P<0.05)。结论:党黄抗癌汤联合肿瘤细胞减灭术中腹腔热灌注治疗Ⅳ期上皮性卵巢癌可有效提高患者免疫力,改善患者生活质量,延长患者生存时间。

Objective:To investigate the effects of Danghuang Kang’ai decoction combined with cytoreductive surgery and intraperitoneal hyperthermic perfusion in the treatment of stage Ⅳ ovarian cancer.Methods:234 cases of stage Ⅳ ovarian cancer patients were randomly divided into observation group and control group,117 cases in each group.The control group was treated with cytoreductive surgery and intraperitoneal hyperthermic perfusion,and the TP method(Docetaxel+Cisplatin) was also given in this group.On the base of control group,the observation group was treated by Danghuang Kang’ai decoction,1 dose/time,1 times/d,the treatment time was 105 days.Before and after the treatment,the scores of emotional state,additional attention,physical condition,social/family status and functional status in 2 groups were evaluated;the levels of CD3+,CD4+,IgA,IgG and IgM were detected.During the treatment,the incidences of toxic effects were observed.After the treatment,2 years follow-up was performed to observe the survival rate of 1 year,2 years,OS and PFS.Results:After the treatment,in the observation group,the scores of emotional state,additional attention,physical condition,social/family status and functional status were(18.35±1.78),(26.78±2.94),(34.28±3.35),(56.21±5.67) and(68.34±6.78),significantly improved than control group(P<0.05);the levels of CD3+,CD4+,IgA,IgG and IgM were(61.11±6.56) %,(46.43±4.75) %,(2.85±0.32) g/L,(15.12±1.65) g/L and(2.39±0.25) g/L,significantly higher than control group(P<0.05).In thelong-time effect evaluation,survival rate of 1 year,2 years,OS and PFS were significantly higher in the observation group than control group(P<0.05).During the treatment,the incidences of toxic effects in the observation group were significantly lower than control group(P<0.05).Conclusion:Danghuang Kang’ai decoction combined with cytoreductive surgery and intraperitoneal hyperthermic perfusion in the treatment of stage Ⅳ ovarian cancer,can improve the immune function and living quality in the patients,lengthen the living time.
