摘要
目的探讨血液透析联合血液灌流对慢性肾衰竭合并骨代谢异常的治疗效果。方法96例慢性肾衰竭合并骨代谢异常患者作为研究对象,根据治疗方式不同分为对照组和观察组,各48例。对照组患者采用血液透析方案治疗,观察组在对照组基础上加用血液灌流方案治疗。比较两组患者临床治疗效果,治疗前后血磷、血钙和甲状旁腺激素水平,治疗前后骨形态发生蛋白(BMP)水平,并发症发生情况。结果观察组患者的治疗总有效率为93.75%,高于对照组的75.00%,差异具有统计学意义(P<0.05)。观察组患者治疗后的血磷水平、血钙水平、甲状旁腺激素水平优于对照组,差异均具有统计学意义(P<0.05)。观察组患者治疗后的BMP3、BMP7和BMP9水平高于对照组,差异均具有统计学意义(P<0.05)。观察组患者并发症发生率为4.17%,低于对照组的20.83%,差异具有统计学意义(P<0.05)。结论血液透析联合血液灌流治疗慢性肾衰竭合并骨代谢异常效果明显,安全性高,有利于改善骨代谢异常,值得在深入探讨的基础上展开广泛应用。
Objective To discuss the therapeutic effect of hemodialysis combined with hemoperfusion on chronic renal failure complicated with abnormal bone metabolism. Methods A total of 96 chronic renal failure patients with abnormal bone metabolism as study subjects were divided into control group and observation group according to different treatment methods, with 48 cases in each group. The control group received hemodialysis regimen for treatment, and the observation group received hemoperfusion regimen for treatment on the basis of the control group. Comparison was made on clinical therapeutic effect, blood phosphorus, blood calcium, parathyroid hormone and bone morphogenetic protein(BMP) levels before and after treatment between the two groups. Results The total effective rate of treatment 93.75% in the observation group was significantly higher than 75.00% in the control group, and the difference was statistically significant(P<0.05). After treatment, the blood phosphorus, blood calcium, parathyroid hormone in the observation group, was better than those in the control group, and the difference was statistically significant(P<0.05). After treatment, the BMP3, BMP7 and BMP9 in the observation group was higher than those in the control group, and the difference was statistically significant(P<0.05). The incidence of complications was 4.17% in the observation group, which was lower than 20.83% in the control group, and the difference was statistically significant(P<0.05). Conclusion Hemodialysis combined with hemoperfusion has obvious effect, high safety in the treatment of chronic renal failure complicated with abnormal bone metabolism, and this method is also conducive to improving abnormal bone metabolism. It is worthy of extensive application on the basis of in-depth discussion.
