小剂量糖皮质激素布地奈德吸入联合甲泼尼龙静脉注射在慢阻肺急性发作治疗中的应用分析

Application of Low-dose Glucocorticoid Budesonide Inhalation Combined with Methylprednisolone in the Treatment of Acute Episodes of Chronic Obstructive Pulmonary Disease

目的研究小剂量糖皮质激素布地奈德联合甲泼尼龙治疗慢阻肺急性发作患者的效果。方法选择该院2016年1月—2018年12月接收的慢阻肺急性发作患者80例,抽签法分为观察组(布地奈德联合甲泼尼龙)与对照组(甲泼尼龙),各40例,比较甲泼尼龙总的使用剂量及时间、血气指标、临床疗效与不良反应。结果两组治疗前血气分析指标差异无统计学意义(P>0.05);治疗后观察组甲泼尼龙使用剂量较对照组少,使用时间较对照组短,PaO2指标较对照组高,PaCO2指标较对照组低,两组组内数据差异有统计学意义(P<0.05);观察组有效率(97.50%)较对照组(80.00%)高(χ^2=6.135,P=0.013);观察组不良反应率(7.50%)较对照组(25.00%)低(χ^2=4.501,P=0.034<0.05)。结论慢阻肺急性发作患者采用小剂量糖皮质激素布地奈德吸入联合甲泼尼龙静脉注射治疗能改善血气分析指标并缩短疗程,提高疗效且减少不良反应,值得借鉴。

Objective To study the effect of low-dose glucocorticoid budesonide combined with methylprednisolone in patients with acute episode of chronic obstructive pulmonary disease.Methods Eighty patients with acute episodes of chronic obstructive pulmonary disease received from January 2016 to December 2018 in the hospital were selected.The sampling method was divided into observation group(Budesonide combined with methylprednisolone)and control group(methylprednisolone),compare the total dose and time of methylprednisolone,blood gas index,clinical efficacy and adverse reactions.Results There were no statistically significant differences in blood gas analysis indexes between the two groups before treatment(P>0.05).After treatment,the dose of methylprednisolone was lower than that of the control group,the use time was shorter than that of the control group,the PaO2 index was higher than the control group,and the PaCO2 index was lower than the control group.The difference was statistically significant between the two groups(P<0.05).The effective rate of the observation group(97.50%)was higher than that of the control group(80.00%)(χ^2=6.135,P=0.013).The adverse reaction rate of the observation group(7.50%)was lower than that of the control group(25.00%)(χ^2=4.501,P=0.034<0.05).Conclusion The low-dose glucocorticoid budesonide inhalation combined with methylprednisolone in patients with acute obstructive pulmonary disease can improve the blood gas analysis index and shorten the course of treatment,improve the curative effect and reduce adverse reactions.It is worth learning.
