血清CA125、CA199联合脂联素对子宫内膜癌的诊断价值

Value Analysis of Serum CA125 and CA199 Combined with Adiponectin in the Diagnosis of Endometrial Carcinoma

目的探讨血清糖类抗原125(CA125)、CA199联合脂联素(APN)检测对子宫内膜癌的诊断价值。方法选取子宫内膜癌患者86例为恶性组,同时期收治的子宫内膜良性病变者54例为良性组,另选取同时期健康体检妇女69例为对照组,对比3组血清CA125、CA199及APN水平,分析CA125、CA199及APN水平与子宫内膜癌患者临床病理参数的关系,并分析CA125、CA199及APN对子宫内膜癌的诊断价值。结果3组受试者血清CA125、CA199及APN水平对比存在显著差异(P<0.05),其中恶性组与良性患者的CA125、CA199水平明显高于对照组,APN水平明显低于对照组(P<0.05),且恶性组患者的CA125、CA199水平明显高于良性组,APN水平明显低于良性组(P<0.05);子宫内膜癌患者血清CA125、CA199水平随着分化程度及临床分期的升高而升高,APN水平随着分化程度及临床分期的升高而降低(P<0.05),不同病理类型子宫内膜癌患者血清CA125、CA199及APN水平对比无明显差异(P>0.05);各指标单一检测对子宫内膜癌的诊断灵敏度、特异度、阳性预测值、阴性预测值对比无明显差异(P>0.05),APN、CA125、CA199联合检测对子宫内膜癌的诊断灵敏度、特异度、阳性预测值、阴性预测值均显著高于单一指标检测(P<0.05)。结论APN、CA125、CA199在子宫内膜癌患者血清中的水平明显升高,且与临床分期及组织分化程度有关,APN、CA125、CA199联合检测可有效提高诊断子宫内膜癌的灵敏度、特异度,具有较高的临床价值。

Objective To investigate the diagnostic value of serum carbohydrate antigen CA125,CA199 combined with adiponectin(APN)in endometrial carcinoma.Methods 86 cases of patients with endometrial carcinoma for malignant group,at the same time the endometrial benign lesions of 54 cases of benign group and 69 cases healthy patients was the control group,compare the 3 groups of serum CA125,CA199 and APN level,analysis of CA125,CA199 and APN levels and clinical pathological parameters,the relationship between endometrial cancer patients and analyze the CA125,CA199 and APN value to the diagnosis of endometrial carcinoma.Results There were significant differences in serum CA125,CA199 and APN levels among the 3 groups.The levels of CA125,CA199 in malignant group and benign group were significantly higher than those of the control group,and the level of APN in malignant group was significantly lower than that of the control group(P<0 05).The CA125,in malignant group was significantly lower than that of the control group(P<0 05).The level of CA199 was significantly higher and the level of APN was significantly lower in benign group than that of benign group(P<0 05).The levels of CA125 and CA199 in the patients with endometrial carcinoma increased with the increase of the degree of differentiation and the clinical stages(P<0.05),and the levels of CA125,CA199 and APN in the patients with different pathological types were not significantly different(P>0.05).The diagnostic sensitivity,specificity,positive predictive value and negative predictive value of each index were no significant difference(P>0.05),APN,CA125 and CA199 in the diagnosis of endometrial carcinoma.Sensitivity,specificity,positive predictive value and negative predictive value were significantly higher than those of single index(P<0.05).Conclusion The level of APN,CA125,CA199 in serum of patients with endometrial carcinoma significantly increased,which was related to clinical stage and histological differentiation.Combined detection of APN,CA125,CA199 could effectively improve the sensitivity and specificity of diagnosis of endometrial carcinoma.It has high clinical value.
