帕瑞昔布钠应用于食管癌单肺通气中对肺分流的影响

Effects of Parecoxib Sodium on Lung Shunt in Single-lung Ventilation of Esophageal Cancer

目的探讨帕瑞昔布钠应用于食管癌单肺通气中对肺分流的影响。方法选择食管癌单肺通气患者133例,根据麻醉药物的不同分为观察组70例与对照组63例。观察组于麻醉前30 min静脉注射帕瑞昔布钠40 mg+生理盐水(10 ml),对照组给予等容量生理盐水,记录2组不同时间点的肺分流情况。结果2组的丙泊酚用量、胶体液输入量、晶体液输入量及术中出血量对比,差异无统计学意义(P>0.05)。2组T0、T1、T2时间点的HR、Sp O2在组内与组间对比,差异都无统计学意义(P>0.05)。观察组T1、T2时间点的肺内分流率显著低于对照组(P<0.05),T0时间点对比差异无统计学意义(P>0.05)。2组T1、T2时间点的MPO与XOD含量都高于T0时间点,观察组低于对照组,对比差异有统计学意义(P<0.05)。结论帕瑞昔布钠应用于食管癌单肺通气中能抑制MPO与XOD的释放,降低肺分流水平,稳定患者的生命体征,有很好的应用价值。

Objective To investigate the effects of parecoxib sodium on lung shunt in single-lung ventilation of esophageal cancer.Methods 133 patients with single-lung ventilation of esophageal cancer were selected and were divided into the 70 patients in the observation group and 63 patients in the control group according to the anesthetic drugs.The observation group were given parecoxib sodium 40 mg+normal saline(10 ml)at 30 min intravenous infusion,the control group were given equal volume of normal saline,and the lung shunts at different time points were recorded.Results There were no significant difference in the amount of propofol,colloidal fluid input,crystal fluid input and intraoperative blood loss compared between the 2 groups(P>0.05).There were no significant differences in the HR and Sp O2 at the time of T0,T1 and T2 compared between the 2 groups(P>0.05).The intrapulmonary shunt rates at the T1 and T2 time points of the observation group were significantly lower than that of the control group(P<0.05),and there were no significant difference in the T0 time point comapred between the 2 groups(P>0.05).The MPO and XOD contents at T1 and T2 were higher than those at T0,and the observation group were lower than the control group,the difference were statistically significant(P<0.05).Conclusion The application of parecoxib sodium in single-lung ventilation of esophageal cancer can inhibit the release of MPO and XOD,reduce the level of pulmonary shunt and stabilize the vital signs of patients,so it has a good application value.
