肌骨超声引导下腔内注射透明质酸钠联合康复运动治疗髋关节炎的临床效果

Effects of ultrasound-guided intra-articular injections combined with exercise training on hip osteoarthritis patients

目的:研究肌骨超声引导下腔内注射透明质酸钠联合康复运动治疗髋关节炎的临床效果。方法:采用随机数字表法将80例确诊为髋关节炎的患者随机分为观察组和对照组,每组40例。观察组行肌骨超声引导下透明质酸钠腔内注射联合康复运动治疗,对照组给予肌骨超声引导下透明质酸钠腔内注射治疗,随访评估治疗效果。分别于治疗前、治疗2周、1个月、3个月时及治疗结束6个月后采用数据评定量表(NRS)和视觉模拟评分法(VAS评分)评估患者髋关节疼痛程度,采用髋关节功能评分(Harris评分)评价患者髋关节功能恢复情况。并随访调查患者对治疗的满意程度和不良反应发生情况。结果:两组患者治疗2周、1个月、3个月时及治疗结束6个月后患者NRS评分均比治疗前降低,差异有统计学意义(P<0.05)。观察组治疗后髋关节疼痛VAS评分优于对照组,差异有统计学意义(P<0.05)。治疗3个月、6个月时,两组患者膝关节功能评分均较治疗前明显改善(P<0.05),且均以观察组的改善幅度较显著,与对照组比较,组间差异均有统计学意义(P<0.05)。治疗3个月后两组均未出现相关并发症,两组患者对治疗的满意程度无显著差异(P>0.05)。结论:肌骨超声引导下腔内注射联合康复运动治疗髋关节炎的临床治疗效果确切,能有效改善髋关节功能,值得进一步研究和推广。

Objective: To evaluate the clinical efficacy of ultrasound-guided intra-articular injections of hyaluronic acid( HA) combined with exercise training for hip osteoarthritis( OA). Methods: A prospective controlled double-blinded randomized trial on 80 patients with hip OA was conducted. The patients were divided in two groups40 patients in each: Observation group underwent intra-articular ultrasound-guided HA injections combined with exercise training,whereas control group underwent intra-articular ultrasound-guided HA injections only. NRS,VAS,and Harris Hip Score were evaluated for both groups before and at 2 weeks,1 month,3 months,and 6 months after treatment. Results: The two groups were comparable in age,sex,body mass index,and severity of hip OA. Both groups showed a significant improvement from baseline at 6-month of follow-ups for all the outcome measures. No major complications were observed during the treatment and follow-ups in both the groups.Conclusion: Ultrasound-guided intra-articular injections of hyaluronic acid( HA) combined with exercise training is a safe and effective procedure,that leads to a relevant ‘improvement’in most patients with hip osteoarthritis( OA).
