超声检查、斑块稳定性指标联合缺血修饰白蛋白评价冠心病进展

Relationship between plaque stability and ischemia modified albumin in patients with coronary heart disease

目的:分析超声评估冠心病患者斑块稳定性联合缺血修饰白蛋白及与疾病病情发展的关系。方法:选择2014年5月至2016年7月在本院就诊的103例疑似冠状动脉粥样硬化心脏病患者作为研究组,另选择同期在本院进行健康体检的健康志愿者50例作为对照组。分别进行颈动脉超声检查,测定两组患者血清缺血修饰蛋白水平。结果:冠心病组ICTP,MMP1,MMP3,MMP7和CatK水平低于对照组,差异有统计学意义(P<0.05)。不同病变程度冠心病患者ICTP,MMP1,MMP3,MMP7和CatK水平差异显著,其中从高至低依次为三支病变患者,双支病变患者、单支病变患者。冠心病组IMT明显大于对照组,IMA水平明显高于对照组,差异有统计学意义(P<0.05)。诊断价值分析结果表明IMT联合缺血修饰蛋白诊断冠心病灵敏度、特异性和准确性分别为0.969、0.961和0.833。结论:超声评估冠心病患者斑块稳定性联合缺血修饰白蛋白与病情进展密切相关,可用于临床早期诊断冠心病。

Objective:To analyze the relationship between plaque stability and ischemia modified albumin in patients with coronary heart disease(CHD).Methods:A total of 103 patients with suspected coronary atherosclerotic heart disease from May 2014 to July 2016 were selected as study group and 50 healthy volunteers as control group.Carotid artery ultrasound was performed to determine the level of serum ischemia modified protein in the two groups.Results:The levels of MMP3 MMP7 and CatK in CHD group were lower than those in the control group(P<0.05).The levels of MMP1 MMP3 MMP7 and CatK in patients with coronary heart disease with different degrees of coronary heart disease were significantly different.From high to low,three-vessel disease patients,two-vessel disease patients and one vessel disease were in turn.IMT in the CHD group was significantly higher than that in the control group(P<0.05).The diagnostic value analysis showed that the sensitivity,specificity and accuracy of IMT combined with ischemic modified protein in the diagnosis of coronary heart disease were 0.961 and 0.833,respectively.Conclusion:Ultrasonic evaluation of plaque stability combined with ischemia modified albumin in patients with coronary heart disease is closely related to the progression of the disease and can be used for early clinical diagnosis of coronary heart disease.
