摘要
目的探讨不同糖化血红蛋白(HbA1c)2型糖尿病(T2DM)患者低、中、高三种负荷条件语言、视空间工作记忆特点。方法将2018年3月-7月在安徽省某三甲医院内分泌科住院的190名T2DM患者按HbA1c四分位数分成HbA1c≤8.1%组、8.1%<HbA1c≤9.25%组、9.25%<HbA1c≤11.3%组和HbA1c>11.3%组,采用N-back范式考查不同HbA1c水平T2DM被试的工作记忆特点。结果不同HbA1c水平被试在低负荷工作记忆任务时,加权反应时无显著差异(P=0.113);HbA1c≤8.1%组被试在三种负荷工作记忆任务时,与其他三组被试的差异无统计学意义(P均>0.05);8.1%<HbA1c≤9.25%组被试在中负荷工作记忆任务时,加权反应时大于9.25%<HbA1c≤11.3%组被试(P=0.008);9.25%<HbA1c≤11.3%组被试在高负荷任务加权反应时大于8.1%<HbA1c≤9.25%组被试(P=0.002);HbA1c>11.3%组被试在中、高负荷工作记忆任务时,加权反应时均最长,在高负荷任务时加权反应时依次大于9.25%<HbA1c≤11.3%和8.1%<HbA1c≤9.25%两组被试(P均<0.05)。结论不同HbA1c水平被试工作记忆表现不同,高负荷任务时HbA1c水平越高工作记忆表现越差。
Objective To explore the working memory characteristics of verbal and visual space of T2DM with different HbA1c.Methods 190 T2DM patients admitted to the department of endocrinology at a first-class hospital in Anhui province from March to July 2018 were divided into HbA1c≤8.1%group,8.1%<HbA1c≤9.25%group,9.25%<HbA1c≤11.3%group and HbA1c>11.3%group according to the quartile of HbA1c level.N-back paradigm was used to examine the working memory characteristics of T2DM among patients with different HbA1c levels.Results There was no significant difference in working memory performance among cases with different HbA1c levels in low-load working memory tasks(P=0.113).There was no significant difference between the HbA1c≤8.1%group and the other three groups in working memory tasks with three loads(all P>0.05).In the 8.1%<HbA1c≤9.25%group,the weighted response time was greater than that in 9.25%<HbA1c≤11.3%group when working memory task was under moderate load(P=0.008).The patients in the group with 9.25%<HbA1c≤11.3%were greater than those in the group with 8.1%<HbA1c≤9.25%,when working memory task was under high load(P=0.002).The cases in the HbA1c>11.3%group had the longest weighted response time in the medium and high workload working memory tasks.In the weighted response time of high-load tasks,it was successively larger than 9.25%<HbA1c≤11.3%and 8.1%<HbA1c≤9.25%(all P<0.05).Conclusion The working memory performance of subjects with different HbA1c was different.And HbA1c level was negatively correlated with working memory performance in high-load tasks.
