摘要
目的探讨负压创面治疗对糖尿病大鼠创面微血管在创面愈合的早期和晚期的改变。方法96只SD大鼠被制作成糖尿病创面模型,并将大鼠分成负压组、纱布组、负压+抑制剂组、纱布+抑制剂组,负压组和负压+抑制剂组采用负压治疗,纱布组和纱布+抑制剂组采用传统纱布治疗,同时负压+抑制剂组和纱布+抑制剂组给予体内注射抑制剂抑制剂,所有实验动物分别在术后1、3、7、10 d取创面肉芽组织行HE染色、免疫荧光检测,并对相关的数据分析。结果在创面愈合的早期阶段(1~3)d,负压创面治疗技术能够促使MVD数量逐渐增加;在创面愈合的晚期阶段(7~10)d,负压创面治疗技术能够显著增加大鼠创面微血管周细胞数量、MVD和MPI。结论负压创面治疗不仅在创面愈合的早期促进微血管环境的不稳定,由此在创面愈合的早期阶段增加血管发生数量,而且在创面愈合的后期能够促进创面血管微环境的稳定,由此促进新血管的血管成熟。并且血管生成素/酪氨酸激酶受体-2信号通路在可能在血管成熟过程中发挥主要作用。
Objective To investigate the effect of negative pressure in wound treatment on the microvascular changes in diabetic rats and to further analyze its underlying molecular mechanism.Methods Ninety-six SD rats were made into a diabetic wound model,and the rats were divided into negative pressure group,gauze group,negative pressure combined with inhibitor group,gauze combined with inhibitor group,negative pressure group and negative pressure combined with inhibitor group.The negative pressure treatment,the gauze group and the gauze combined with inhibitor group were treated with traditional gauze,while the negative pressure combined with inhibitor group and gauze combined with inhibitor group were given inhibitor of injections in vivo.All the experimental animals was taken wound granulation tissue to do HE staining and immunofluorescence detection after 1,3,7,10 days,And related data were analyzed.Results In the early stage of wound healing(for 1-3 days),the treatment of negative pressure wound can promote the increase of MVD.In the later stage of wound healing(for 7-10 days),negative pressure wound treatment can significantly increase the number of wound microvessel count,MVD and MPI of rats.Conclusions Negative pressure in wound therapy not only promotes the instability of microvascular environment in the early stage of wound healing,thus increasing the number of angiogenesis in the early stage of wound healing,but also promots the stability of wound vascular microenvironment in the later stage of wound healing,thereby promoting neovascularization of blood vessels.The angiopoietin/tyrosine kinase receptor-2 signaling pathway plays a major role in regulating vascular maturation.
