某高校教工血尿酸水平与糖尿病发病关系的研究 被引量:1

摘要 目的探讨北京某高校教工血尿酸水平与糖尿病发病的关系。方法2006-2016年期间,对参加过2006年教工健康体检的人中空腹血糖<7.0 mmol/L、无糖尿病病史且未使用降糖药物的教工进行随访观察,到2016年底,共计纳入1595名教工作为研究对象。根据研究对象的性别、高血压患病情况、血尿酸水平进行分组,分析不同血尿酸水平对新发糖尿病的影响。结果非高血压女性教工中,不同尿酸组教工的血压,体质指数(BMI),血糖,血脂水平比较,除了高密度脂蛋白胆固醇(HDL-C),其他指标的差异均有统计学意义(P<0.05);高血压女性教工中,不同尿酸组教工的甘油三酯(TG),HDL-C水平比较,差异均有统计学意义(P<0.05)。非高血压男性教工中,不同尿酸组教工的BMI,血清胆固醇(TC),TG及HDL-C水平比较,差异均有统计学意义(P<0.05);高血压男性教工中,不同尿酸组教工的BMI,TG水平比较,差异均有统计学意义(P<0.05)。截至2016年底,研究对象中有78例新发糖尿病,发病率为5.00/1000人年。女性非高血压组和男性非高血压组人群从尿酸第2组(女性:尿酸186~<218μmol/L;男性:尿酸233~<274μmol/L)开始,随着尿酸水平的逐渐升高,糖尿病的发病率也随之升高,但差异均无统计学意义(P>0.05)。此外,女性高血压组和男性高血压组人群中,不同尿酸水平研究对象糖尿病发病率的差异也无统计学意义(P>0.05),但在高血压患者中,不论男性或者女性,当其血尿酸水平在第3组(女性:218~<249μmol/L;男性:274~<313μmol/L)时,糖尿病的发病率最高,男女发病率分别为18.81/1000人年,17.00/1000人年。结论当女性高血压患者血尿酸水平为218~<249μmol/L,男性高血压患者血尿酸水平为274~<313μmol/L时,其糖尿病的患病风险可能会增加。 Objective To investigate the relationship between blood uric acid level and diabetes incidence among faculty members of a college in Beijing.Methods From 2006 to 2016,follow-up observations were performed on the faculty members who had participated in the 2006 physical examination with a fasting blood glucose<7.0 mmol/L,no history of diabetes and had not used hypoglycemic drugs.By the end of 2016,a total of 1595 subjects were enrolled.The subjects were divided into several groups according to their gender,the prevalence of hypertension and the level of serum uric acid,and the effects of different blood uric acid levels on new-onset diabetes were analyzed.Results Among the groups of non-hypertensive female subjects with different uric acid levels,the differences in all the indices of blood pressure,body mass index(BMI),blood glucose and blood lipid levels were statistically significant except that of high-density lipoprotein cholesterol(HDL-C)(P<0.05);among the hypertensive female subjects,the differences in triglyceride(TG)and HDL-C levels in different uric acid groups were statistically significant(P<0.05).Among the non-hypertensive male subjects,the differences in BMI,serum cholesterol(TC),TG and HDL-C levels in different uric acid groups were statistically significant(P<0.05);among the hypertensive male teachers,the difference in the BMI and TG levels in different uric acid groups were statistically significant(P<0.05).As of the end of 2016,there were 78 new cases of diabetes in the study,with an incidence of 5.00/1000 person-years.Starting from uric acid group 2(female:uric acid 186 to<218μmol/L;male:uric acid 233 to<274μmol/L)of non-hypertensive subjects,with the gradual increase of uric acid level,the diabetes incidence rate also increased,but the difference was not statistically significant(P>0.05).In addition,there was no significant difference in the incidence of diabetes among hypertensive subjects in different uric acid groups(P>0.05),but among the hypertensive subjects,male and female alike,the diabetes incidence was highest in uric acid group 3(female:218 to<249μmol/L;male:274 to<313μmol/L),which were 18.81/1000 person-years(male)and 17.00/1000 person-years(female),respectively.Conclusion When the blood uric acid level is 218 to<249μmol/L in female hypertensive patients and 274 to<313μmol/L in male hypertensive patients,the risk of diabetes may increase.
