首发精神分裂症患者认知障碍与比率低频振幅的相关研究 被引量:8

The correlation between cognitive function and fraction amplitude of low-frequency fluctuation of patients with first -episode schizophrenia

摘要 目的通过静息态功能磁共振(functional MRI,fMRI)比率低频振幅(fraction amplitude of low frequency fluctuation,fALFF)方法探讨首发精神分裂症患者认知功能障碍的潜在机制。方法纳入28例首发精神分裂症患者(first-episode schizophrenia,FSZ)和33名年龄、性别、受教育年限相匹配的对照者,用重复性成套神经心理状态测验(repeatable battery for the assessment of neuropsychological status,RBANS)评估受试者的认知功能,进行静息态fMRI扫描并采用f ALFF方法分析两组差异脑区,提取患者差异脑区内的fALFF值分析与认知功能评分的相关性。结果两组即刻记忆、视觉广度、言语功能、注意力、延时记忆的差异均有统计学意义(P<0.05)。患者双侧中央旁小叶、右侧辅助运动区、左侧中央后回、左侧颞上回、左侧中央沟盖的fALFF值较对照组降低(P<0.01,FDR校正),未发现f ALFF值升高的区域。相关分析中,患者组左侧中央后回fALFF值与即刻记忆得分呈负相关(r=-0.39,P=0.04)。结论首发精神分裂症患者认知功能全面受损,多处脑区自发活动降低,且部分脑区的异常自发活动与认知功能评分相关,这可能为精神分裂症认知功能障碍潜在机制提供参考。 Objective A full picture of resting-state functional magnetic resonance imaging(fMRI) alteration and cognitive function in population with first-episode schizophrenia is important to understand the insights of schizophrenia which however remains largely under-studied.In the present study, we examined the cognitive function in Chinese Han patients with first-episode schizophrenia in fraction amplitude of low frequency fluctuation(fALFF).Methods Twentyeight patients with first-episode schizophrenia(FSZ) and 33 age, gender and education matched healthy controls(HC)were recruited in this study.A1 l subjects underwent resting-state fMRI.The fALFF method were selected and fALFF value were extracted from brain regions of interest, while the repeatable battery for the assessment of neuropsychological status(RBANS) were selected to evaluate the cognitive function.Correlations between fALFF value and cognitive function were further performed.Results Differences in immediate memory, visual span, speech function, attention and delayed memory were statistically significant between the two groups(P<0.05).The fALFF values: compared with the HC subjects,reduced fALFF value were distributed in regions including the bilateral paracentral lobule lobes, the left postcentral gyrus, the left superior temporal, the supplementary motor area, and the right rolandic operculum in patients with FSZ( P<0.01, FDR corrected).However, no region with elevated fALFF was observed in FSZ subjects.Correlation analysis showed that fALFF value in the left postcentral was negatively correlated with the immediate memory scores in RBANS test(r=-0.39,P=0.04).Conclusion Our study indicates that the correlation between abnormal spontaneous activity of Chinese Han FSZ and the deficit of certain cognitive ability, which may provide a reference for the cognitive mechanism of schizophrenia.
