温阳化气法治疗肾阳虚型精癃临床疗效观察

The clinical effect of warming Yang and resolving Qi on benign prostatic hyperplasia of kidney Yang deficiency type

目的:探讨温阳化气法治疗肾阳虚型精癃的临床疗效。方法:选取2016年5月-2018年5月于山西中医学院附属医院就诊的64例肾阳虚型精癃患者作为研究对象,随机分为治疗组和对照组各32例,治疗组予温阳化气法治疗,对照组予盐酸坦索罗辛缓释胶囊、非那雄胺片治疗,两组疗程均为1个月。对比分析两组治疗后总有效率,观察患者治疗前后国际前列腺症状评分(IPSS)、生活质量指数评分(QOL)、最大尿流率(Qmax)、残余尿量(PVR),观察治疗组治疗前后中医证候评分,记录两组不良反应发生情况。结果:治疗组总有效率为90.6%,高于对照组81.3%(P<0.05);两组治疗后IPSS评分、QOL评分、PVR均下降,Qmax上升,与治疗前比较,差异具有统计学意义(P<0.05);治疗后治疗组IPSS评分、QOL评分、PVR均低于对照组,Qmax高于对照组,差异具有统计学意义(P<0.05);治疗后,治疗组中医证候评分明显下降,与治疗前比较差异具有统计学意义(P<0.05)。治疗组未见不良反应,对照组反馈2例性欲减退。结论:采用温阳化气法治疗肾阳虚型精癃疗效可靠,可明显改善患者的临床症状。

Objective:To observe the clinical effect of treatment method of warming Yang and resolving Qi on benign prostatic hyperplasia of kidney Yang deficiency type.Methods:64 patients with benign prostatic hyperplasia of kidney-Yang deficiency type who were treated in the affiliated hospital of Shanxi college of TCM from May 2016 to May 2018 were selected as subjects of study.64 patients were randomly divided into treatment group and control group,50 cases in each group.The treatment group was treated by method of warming Yang and resolving Qi,while the control group was treated by tamsulosin hydrochloride sustained-release capsules and finasteride tablets for 30 days.The total effective rates were calculated,international prostate symptom scale(IPSS),quality of life index(QOL),maximum urinary flow rate(Qmax)and residual urine volume(PVR)were observed before and after treatment.The scores of TCM symptoms before and after treatment in the treatment group were observed,and the adverse reactions in the two groups were recorded.Results:After treatment,the total effective rate was 90.6%in treatment group and 81.3%in control group respectively(P<0.05).Compared with before treatment,the scores of IPSS and QOL、PVR were decreased,Qmax was increased in two groups(P<0.05).After treatment,the scores of IPSS and QOL、PVR in treatment group was less than those in control group,Qmax was higher than that in control group(P<0.05).TCM syndrome score in treatment group was significantly higher than that of before treatment.Conclusion:The methods of warming Yang and resolving Qi has significant clinical effect in treating benign prostatic hyperplasia of kidney-Yang deficiency type.The method could effectively improve the symptoms of patients without adverse reaction,so it is worthy of promotion and application in clinical practice.
