双管喉罩与气管插管用于全麻妇科腹腔镜手术的效果比较

Comparison of the Effects of Double-tube Laryngeal Mask and Tracheal Intubation for General Anesthesia Gynecological Laparoscopic Surgery

目的比较妇科腹腔镜手术全身麻醉采用双管喉罩与气管插管的临床效果。方法方便择取2016年7月—2019年2月该院妇科78例腹腔镜手术患者,分为实验组(双管喉罩通气,39例)和对照组(气管插管通气,39例),比较两组术中基本生命指标、血气分析结果与通气相关并发症。结果实验组喉罩置入一次成功率(97.44%)与对照组气管插管(100.00%)差异无统计学意义(χ^2=2.517,P>0.05),前者术中生命体征较后者更平稳。实验组术中PO2(87.1±6.3)mmHg,PCO2(42.7±3.0)mmHg,通气相关并发症(7.69%),与对照组比较差异有统计学意义(t=4.581,t=3.162,χ^2=6.307,P<0.05)。结论双管喉罩用于全麻妇科腹腔镜手术应激反应小,并发症少,更具临床优势,但要注意对血氧的影响。

Objective To compare the clinical effects of double-tube laryngeal mask and tracheal intubation in general anesthesia for gynecological laparoscopic surgery.Methods Convenient select from July 2016 to February 2019,78 patients with laparoscopic surgery in our hospital were divided into experimental group(dual laryngeal mask ventilation,39 cases)and control group(tracheal intubation ventilation,39 cases).The basic life indicators,blood gas analysis results and ventilation related complications in the two groups.Results There was no statistically significant of the difference in the success rate of the laryngeal mask placement in the experimental group(97.44%)and the tracheal intubation in the control group(100.00%)(χ^2=2.517,P>0.05).The vital signs of the former were more stable than the latter.In the experimental group,PO2(87.1±6.3)mmHg,PCO2(42.7±3.0)mmHg,and ventilation-related complications(7.69%)were statistically different from the control group(t=4.581,t=3.162,χ^2=6.307,P<0.05).Conclusion Double-tube laryngeal mask for general anesthesia gynecological laparoscopic surgery has less stress response,less complications and more clinical advantages,but attention should be paid to the effect on blood oxygen.
