急诊胃镜在消化道出血的应用价值

The value of emergency gastroscopy in gastrointestinal bleeding

目的探究急诊胃镜在上消化道出血中的应用价值。方法选取本院2016年1月至2018年12月收治上消化道出血患者900例,据胃镜检查时间分组,出血24 h内接受胃镜检查者为急诊A组(354例),出血24~48 h接受胃镜检查者为急诊B组(305例),出血48 h后接受胃镜检查者为择期组(241例)。比较3组患者止血率、再出血率、并发症发生率、死亡率、输血量及住院时间。结果止血率急诊A组>急诊B组>择期组(P<0.05);再出血率、并发症发生率、输血量、住院时间急诊A组<急诊B组<择期组(P<0.05);死亡率急诊A组低于择期组(P<0.05)。结论急诊胃镜有利于提高上消化道出血患者的治疗效果,缩短患者住院时间。

Objective To explore the value of emergency gastroscopy in upper gastrointestinal bleeding.Methods A total of 900 patients with upper gastrointestinal bleeding from January 2016 to December 2018 in the hospital were enrolled.According to the time of gastroscopy,patients undergoing gastroscopy within 24 hours of bleeding were in emergency group A(354 cases).Patients undergoing gastroscopy within 24 h to 48 h of bleeding were in the emergency group B(305 cases).Patients undergoing gastroscopy at 48 hours after bleeding were in elective group(241 cases).The hemostatic rate,rebleeding rate,occurrence rate of complication,mortality,blood transfusion volume and hospitalization time were compared between the three groups.Results Arranging the hemostatic rate from high to low,the corresponding order was emergency group A,emergency group B and elective group(P<0.05).Arranging the rebleeding rate,occurrence rate of complication,blood transfusion volume and hospitalization time from low to high,the corresponding order was emergency group A,emergency group B and elective group(P<0.05).The mortality rate in emergency group A was lower than that in the elective group(P<0.05).Conclusion Emergency gastroscopy is beneficial to improve the treatment effect of patients with upper gastrointestinal bleeding and shorten the hospital stay.
