血清降钙素原联合病原微生物筛查在学龄前儿童下呼吸道感染中的临床作用 被引量:3

摘要 目的探讨血清降钙素原(PCT)联合病原微生物筛查在学龄前儿童下呼吸道感染中的临床作用。方法选取本院2018年6月至2018年12月确诊的学龄前儿童下呼吸道感染患儿80例,根据细菌培养结果将其分为细菌感染患儿(n=42)和非细菌感染患儿(n=38),选取同期健康体检的学龄前儿童32例作为参照,全部入选儿童均采集血液样本、痰标本,检测受试儿童体内PCT水平。将80例下呼吸道感染患儿随机分为对照组和观察组,各40例,对照组采用常规抗感染治疗,观察组根据检测结果对症给予患儿抗生素治疗,对比治疗效果。结果感染组、非感染组的学龄前儿童下呼吸道感染PCT水平和阳性率与健康组比较差异具有统计学意义(P<0.05);观察组和对照组患儿的退热时间和住院时间比较差异无统计学意义,观察组抗生素联合使用率(27.50%)明显低于对照组(50.00%)(P<0.05);不同指标诊断新生儿感染的敏感性、特异性、准确性、漏诊率和误诊率依次为PCT:85.7%、84.2%、85.0%、14.3%、15.8%;IgM:76.2%、81.6%、78.8%、23.8%、18.4%;PCT联合IgM:95.2%、94.7%、95.0%、4.8%、5.3%。结论细菌组感染患儿、非细菌感染患儿和健康儿童的PCT水平差异具有统计学意义;采用PCT联合IgM筛查能够评估患儿病况,准确鉴别细菌感染患儿,对合理使用抗生素、判断患儿病情具有积极的指导意义。
