慢性肾脏病肾虚湿瘀病机科学内涵及益肾清利活血法的作用机理

Scientific Connotation of Chronic Kidney Disease with Kidney Deficiency and Dampness Stasis and the Mechanism of Yishen Qingli Huoxue Therapy

慢性肾脏病(chronic kidney disease,CKD)是临床常见病,在现今老龄化社会发病率逐年升高。孙伟教授认为“肾虚湿瘀”是慢性肾脏病的基本病机,而“益肾清利活血法”是孙伟教授长期实践于临床的慢性肾脏病中药汤剂治法。孙伟教授科研团队历时10余年,通过阿霉素大鼠模型、糖尿病肾病大鼠模型、UUO模型等,证实了特定中药对大鼠肾损伤的改善作用。我们发现中药对于肾损伤信号通路的调控作用(自噬信号通路、AMPK通路、NLRP3炎症小体)。本文探讨了“肾虚血瘀”病机的科学内涵和“益肾清利活血法”的作用机理,为益肾清利活血法治疗慢性肾脏病在临床的应用提供了支撑。

The continuous progress of chronic kidney disease(CKD)is an unresolved clinical problem,and the TCM treatment is effective in delaying the progression of renal function.Yishen Qingli Huoxue Therapy is a traditional Chinese medicine decoction treatment method used by Professor Sun Wei for long-term practice in patients with kidney deficiency and dampness stasis syndrome.Professor Sun Wei's research team,with the support of 3 national natural funds,has established a rat model of doxorubicin,a rat model of diabetic nephropathy,a UUO model,etc.Through the measurement of proteinuria and creatinine urea nitrogen in rats with nephropathy,study on pathological damage,ischemic conditions,and related signaling pathways-autophagy signaling pathway,energy metabolism AMPK pathway,NLRP3 inflammatory body activation and other comprehensive research,the pathogenesis and drug action mechanism was studied.This paper partially clarified the scientific connotation of kidney deficiency and blood stasis and the mechanism of Yishen Qingli Huoxue Therapy,and provides support for the clinical application of Yishen Qingli Huoxue Therapy in the treatment of chronic kidney disease.
