八段锦和抗阻运动对糖调节受损人群血糖和胰岛素抵抗的影响 被引量:21

Effects of Ba Duan Jin and Resistance Exercises on Glucose and Insulin Resistance in Patients with Impaired Glucose Regulation

摘要 目的研究不同的运动方式对糖调节受损人群血糖和胰岛素抵抗指数的影响。方法共纳入符合标准的受试者152例,随机分为八段锦组、抗阻运动组和对照组,经过9个月的干预和随访,最终收集141例完整数据。于干预前和干预后的3、6、9月抽血,查空腹血糖(fasting plasma glucose,FPG)和空腹胰岛素(fasting blood insulin,FINS),并计算胰岛素抵抗指数(homeostatic model assessment for insulin resistance,HOMA-IR)和胰岛素作用指数(Insulin activity index,IAI)。结果①不同时间点之间的FPG、FINS、HOMA-IR、IAI比较,差异有统计学意义(F=373.463,332.171,685.646,467.288,均P=0.000);3组间4项指标比较,差异有统计学意义(F=64.190,6.993,22.637,24.365,P=0.000、0.001、0.000、0.000),干预后八段锦组和抗阻运动组的FPG、FINS、HOMA-IR低于对照组,IAI高于对照组;3组的4项指标变化趋势差异有统计学意义(F=111.660,100.695,208.643,129.599,均P=0.000),干预期间,对照组的FPG、FINS、HOMA-IR总体呈上升趋势,IAI呈下降趋势;两运动组的FPG、FINS、HOMA-IR总体呈下降趋势,IAI呈上升趋势。而两运动组在各时间点的检测结果比较差异不明显,无统计学意义(P>0.05);②干预3、6、9个月时,八段锦组和抗阻运动组的FPG、FINS、和HOMA-IR均低于干预前,IAI均较干预前更高,差异显著(P<0.01)。结论八段锦和抗阻运动对IGR患者的空腹血糖、胰岛素抵抗和胰岛素敏感性均有显著的改善,作用相似,而八段锦在患者依从性方面具有优势,值得推广。 Objective:To study the effects of different exercise modes on glucose and insulin resistance in patients with impaired glucose regulation(IGR).Methods:A total of 152 subjects who met the criteria were randomly divided into a Ba Duan Jin group,a resistance movement group and a control group.After 9 months of intervention and follow-up,141 cases of complete data were finally collected.Blood samples were taken before and 3,6,9 months after the intervention,fasting plasma glucose(FPG)and fasting insulin(FINS)were checked,and insulin resistance index(HOMA-IR)and insulin action index(IAI)were calculated.Results:Comparison of FPG,FINS,HOMA-IR and IAI of different time points showed statistically significant differences(F=373.463,332.171,685.646,467.288,all P=0.000).Comparison of 4 indexes among the three groups showed statistically significant differences(F=64.190,6.993,22.637,24.365,P=0.000,0.001,0.000,0.000,and 0.000).After the intervention,FPG,FINS and HOMA-IR of the Ba Duan Jin group and the resistance exercise group were lower than the control group and IAI was higher than the control group.The variation trend differences of 4 indexes of the 3 groups were statistically significant(F=111.660,100.695,208.643,129.599,all P=0.000).During the intervention period,FPG,FINS and HOMA-IR of the control group were generally on the rise,while IAI was on the decline.Overall,FPG,FINS and HOMA-IR of the two sports groups presented a downward trend,while IAI presented an upward trend.Among them,there was no significant difference in the indexes between the 2 groups at different time periods(P>0.05).At the end of 3,6,and 9 months after intervention,FPG,FINS,and HOMA-IR of Ba Duan Jin group and resistance exercise group were lower than those before intervention,and IAI were higher than those before intervention,with statistically significant differences(P<0.01).Conclusion:Ba Duan Jin and resistance exercise had significantly improved FPG,insulin resistance and insulin sensitivity in IGR patients,and the effects are similar,while Ba Duan Jin has advantages in patient compliance and is worth promoting.
