医院-社区-家庭延续护理模式在脑卒中患者康复护理中的应用

目的探讨医院-社区-家庭延续护理模式在脑卒中患者康复护理中的应用效果。方法选取2017年7月至2018年8月在巴州人民医院神经内科就诊的脑卒中患者80例作为研究对象,所有患者按照随机数字表法分为两组,各40例。对照组患者给予常规护理,观察组则在常规护理的基础上给予医院-社区-家庭延续护理模式。对比两组干预前后日常生活能力、肢体功能评分及并发症发生情况。结果两组干预前日常生活能力及肢体功能评分比较,差异无统计学意义(P>0.05);与对照组相比,观察组出院时及干预6个月后日常生活能力和肢体功能评分均较高,并发症发生率较低,差异有统计学意义(P<0.05)。结论对脑卒中患者实施医院-社区-家庭延续护理,较好的保证了护理的延续性,利于改善患者的日常生活能力和肢体功能,降低并发症发生率,值得临床推广应用。

Objective To explore the application effect of Hospital-Community-Family continuous nursing mode in rehabilitation nursing of stroke patients.Methods Eighty patients with stroke in the department of neurology,Bazhou People's Hospital from July 2017 to August 2018 were enrolled.All patients were divided into two groups according to the random number table method,40 cases each.Patients in the control group were given routine care,while the observation group was given a Hospital-Community-Family continuation care mode based on routine care.The daily living ability,limb function score and complications were compared before and after intervention.Results There was no significant difference in ADL and limb function scores between the two groups before intervention(P>0.05);compared with the control group,the observation group had higher ADL and limb function scores at discharge and 6 months after intervention,and the incidence of complications was lower(P<0.05).Conclusion The implementation of Hospital-Community-Family continuous nursing mode for stroke patients can better ensure the continuity of nursing,improve the daily living ability and limb function of patients,and reduce the incidence of complications.It is worthy of clinical application.
