Therapy and prevention of bladder cancer by Bacillus Calmette-Guerin vaccine for treatment

摘要 目的探讨治疗用卡介苗(Bacillus Calmette-Guerin vaccine for treatment,BCGt)在膀胱癌预防及治疗中的作用。方法MB49细胞灌注小鼠膀胱,建立小鼠原位膀胱癌模型,进行BCGt预防性和治疗性试验,观察小鼠生存情况。末次治疗后第6天,经小鼠眼球采血,检测血清中IgG、IFNγ和IL-6的含量,同时取小鼠脾脏,检测特异性T淋巴细胞;每次治疗后检测小鼠尿液中一氧化氮(NO)含量;末次治疗45 d后进行记忆性免疫试验。结果成功建立了基于MB49细胞的小鼠原位肿瘤模型,致癌率100%。BCGt免疫预防和治疗两种方式均可延长小鼠的生存期;BCGt治疗后小鼠血清中的IgG、IFNγ、IL-6及尿液中NO含量明显升高(P<0.05),特异性T淋巴细胞反应明显增强(P<0.001),且可产生免疫记忆。结论BCGt具有预防及治疗膀胱癌的作用,且BCGt抗肿瘤作用可能是通过产生IFNγ、IL-6、NO等发挥作用的。 Objective To preliminarily explore the mechanism of Bacillus Calmette-Guerin vaccine for treatment(BCGt)in the treatment of bladder cancer and the prevention of recurrence.Methods A mouse model of bladder carcinoma in situ was established based on MB49 cell perfusion.Prevention and treatment experiments were performed with BCGt,and the murine survival was documented.On the 6th day post BCGt,boold samples were collected from murine eyes,from which levels of IgG,IFNγ,and IL-6 were measured.Murine spleens were also harvested and tested for tumor-specific T lymphocyte response.NO levels in the murine urea were measured after each BCGt treatment.Immune memory was qualified 45 d after the last BCGt treatment.Results The murine in situ tumor model based on MB49 cells was successfully established with MB49 cell with a 100%carcinoma development rate.Both the immunoprophylaxis of BCGt before inoculation with MB49 cells and the immunotherapy of BCGt after inoculation MB49 cells could prolong the survival period of mice.After trentment with BCGt,the IgG,IFNγand IL-6 levels in serum and NO level in murine urea increased significantly(P<0.05),while the specific T lmphocyte response level increased significantly(P<0.001),and immune memory was induced.Conclusion The in situ bladder cancer model of mice established in this study has confirmed the anti-tumor efficacy of BCGt,possibly by increasing levels of IFNγ,IL-6 and NO.
