郑州市2011—2016年国家基本公共卫生服务项目公平性分析

Equity analysis of implementation effect of national basic public health service projects in Zhengzhou,2011-2016

目的研究2011-2016年郑州市国家基本公共卫生服务项目实施效果公平性,为进一步促进其顺利实施提供参考依据和可行性建议。方法采用基尼系数,从人口服务公平性和经济服务公平性角度评价郑州市国家基本公共卫生服务项目实施效果。结果 2011-2016年,郑州市大部分国家基本公共卫生服务项目的基尼系数稳定在0.2~0.3之间,且呈下降趋势,其中0~6岁儿童管理人数和65岁以上老年人建档数的基尼系数一直低于0.2,处于高度平均状态。重症精神病管理服务常在警戒线附近徘徊,公平性较差;传染病及时上报的病例数和卫生监督协管服务的基尼系数值均大于0.5,处于高度不平均状态。结论 2011-2016年,郑州市国家基本公共卫生服务项目基本实现均等化,但重症精神病管理服务与卫生监督协管服务的公平性有待提高,需加强政府财政和相关政策的支持,对重点地区、薄弱项目加大投入。

Objective To analyze the implementation effect of national basic public health service projects in Zhengzhou from 2011 to 2016,provide reference basis and feasible suggestions for further promoting its implementation.Methods The Gini coefficient was used to evaluate the implementation effect of Zhengzhou national basic public health service project from the perspective of population service equity and economic service equity.Results From 2011 to 2016,the Gini coefficient of most national basic pubic health service projects in Zhengzhou stabilized between 0.2 and 0.3 with a decline trend.Among them,the Gini coefficient of the number of children aged 0~6 and the number of elderly people aged 65 and above had been below 0.2,which were in high average state.However,the Gini coefficient of the severe psychiatric management services often wandered around the warning line,which was less equitable.In the past six years,the Gini coefficient of the number of cases of infectious diseases reported in time and the Gini coefficient of health supervision and management services were all over 0.5,which was in a highly unequal state.Conclusion The national basic public health service projects in Zhengzhou were basically equalized from 2011 to 2016.But the fairness of critical psychiatric management services and health supervision and management services needed to be improved.The government should strengthen financial and related policy support,and increase investment in key areas and weak projects.
