针药结合治疗颈源性头痛疗效观察

Clinical Observation of Acupuncture and TCM Medication in Treating Cervicogenic Headache

目的:观察排针平刺配合口服头痛新1号方治疗颈源性头痛的疗效。方法:96例颈源性头痛患者随机分为针刺组、中药组和针药组,每组32例。针刺组采用排针平刺(双侧脑户透风府、脑空透风池,留针6 h,隔日1次,每周3次,共2周);中药组采用口服头痛新1号方(每日1剂,每周7剂,共2周);针药组采用针刺(同针刺组)配合中药(同中药组)。观察治疗前后3组患者疼痛视觉模拟量表(VAS)评分及颈椎关节活动度(ROM)评分,并记录不良反应。结果:针药组、针刺组、中药组治疗后VAS、ROM评分均显著低于治疗前(P<0.05);针药组治疗后VAS、ROM评分均显著低于针刺组、中药组(P<0.05);针刺组与中药组治疗后VAS、ROM评分相比不具有统计学差异(P>0.05)。针药组总有效率优于针刺组、中药组(P<0.05),且3组治疗均无严重不良反应。结论:运用排针平刺法针刺配合口服头痛新1号方治疗颈源性头痛可有效改善患者头痛及颈椎关节活动受限情况,安全性良好,且优于单纯运用排针平刺法针刺或口服头痛新1号方治疗。

Objective:To observe the effect of parallel transverse needling combined with Toutongxin No.1 in treating cervicogenic headache.Methods:96 patients with cervicogenic headache were randomly divided into the acupuncture group,the TCM medication group and the combination group(acupuncture+TCM medication),with 32 cases in each group.Parallel transverse needling GB19-to-GB20 and DU17-to-DU16 was applied in the acupuncture group,with 6-hour needle retention,the treatment was once every 2 days for 2 weeks.The TCM medication group was treated with Toutongxin No.1(one dose per day,7 times per week for 2 weeks).The combination group was treated with both therapies as above.The scores of visual analogue scale(VAS),the scores of range of motion(ROM)and side effects were observed before and after the treatment in the three groups.Results:The scores of VAS and the scores of ROM were significantly lower after the treatment than those before the treatment in the three groups(P<0.05);of which the combination group improved more significantly than the other two groups(P<0.05),but there was no statistical difference between the other two groups(P>0.05).The total effective rate of the combination group was superior to that of the other two groups(P<0.05).There were no serious adverse reactions in the three groups.Conclusion:Parallel transverse needling combined with Toutongxin No.1 is safe and effective for cervicogenic headache,it can effectively alleviate headache and improve the motion of cervical vertebra.The therapy is superior to single acupuncture treatment or single TCM medication.
