观察针刺手少阴心经穴不同时间段对脑梗死恢复期患者ROI-CBF的影响

Effect of Needling Points of Heart Meridian on ROI-CBF in Patients during Cerebral Infarction Convalescence

目的:观察针刺手少阴心经穴不同时间段对脑梗死恢复期患者各感兴趣区(ROI)脑血流量(CBF)的影响。方法:选取符合纳入标准的脑梗死恢复期患者13例,以脑梗死恢复期患者梗死灶、梗死灶旁及对侧镜像区为感兴趣区,以ROI-CBF为观察指标,采用MR ASL扫描,获取针刺前、针刺即刻及拔针后3个时间段各ROI-CBF并分析其差异。结果:心经穴能够提高各ROI-CBF(P<0.05),其中拔针后时间段尤为明显;各ROI-CBF值增加幅度依次为:镜像区≈梗死灶旁区>梗死灶区(P<0.05)。结论:针刺心经穴能有效提高梗死灶区、梗死灶旁区、镜像区CBF,为临床上选取心经穴治疗缺血性脑血管疾病提供了部分可视化依据。在同一时间段下,针刺心经穴对梗死灶区、梗死灶旁区、镜像区CBF存在差异。

Objective:To observe the effect of needling points of Heart meridians on brain blood flow(CBF)in various region of interest(ROI)during the convalescence of cerebral infarction.Methods:13 patients during cerebral infarction convalescent period who met the criteria were included.The infarction location,the area beside infarction and contralateral mirror area were taken as ROI,and ROI-CBF was used as observation indicator.Magnetic resonance artery spin labeling(MR ASL)scanning was used to detect the differences of ROI-CBF before needling,immediately after needling and after withdrawal of needles.Results:Needling points of Heart meridian could improve ROI-CBF at different time point(P<0.05),especially after needle withdrawal.The increase value of ROI-CBF in the mirror area and in the area beside infarction were similar,which were higher than that in the infraction location(P<0.05).Conclusion:Needling points of Heart meridian can effectively improve CBF in the infarction area,the area beside infraction and the mirror area,which provides visual basis for clinical selection of Heart meridian points in the treatment of ischemic cerebrovascular disease.There are differences in CBF at the same time point of needling among the infarction area,the area beside infarction and the mirror area.
