全自动血细胞分析仪与血涂片细胞形态学在血常规检验中的联合应用分析 被引量:13

摘要 目的探讨全自动血细胞分析仪联合血涂片细胞形态学检验方法在血常规检验中的应用价值。方法对2018年3-10月该院检验科接收的体检者血液标本84例,分别采取全自动血细胞分析仪和血涂片形态分析法进行检验,比较血细胞分析仪联合血涂片镜检法、单纯血细胞分析仪检验法的血常规检验结果。结果①84份血样中,经血细胞分析仪检验报警40份,阳性检出率为47.6%(40/84),联合检验共报告血常规异常55份,阳性检出率为65.5%(55/84),经比较,两种血常规检验方法阳性检出率差异有统计学意义(χ^2=4.748,P=0.029<0.05);②84份样本的血细胞分析结果显示,WBC、PLT、NE、LY、EO、MO几项血常规指标检出阳性分别有38例、24例、30例、18例、2例和4例,阳性率分别为45.2%、28.6%、35.7%、21.4%、2.4%和4.8%;联合检验结果显示检出阳性分别有40例、26例、33例、19例、10例和13例,阳性率分别为47.6%、30.9%、39.3%、22.6%、11.9%和15.5%,其中两组检查结果中EO、MO阳性检出率比较,差异有统计学意义(EO:χ^2=4.397,P=0.036;MO:χ^2=4.188,P=0.041<0.05),其余项目差异无统计学意义(P>0.05)。结论在血常规检验中联合应用血细胞分析技术、血涂片细胞形态学检验技术,有助于提高血常规异常样本的检出率,弥补血细胞技术在细胞形态学检验方面的不足。 Objective To explore the application value of automatic blood cell analyzer combined with blood smear cell morphology test in blood routine test. Methods 84 blood samples from the physical examination subjects received by the laboratory from March to October 2018 were tested by automatic blood cell analyzer and blood smear morphology analysis. The blood cell analyzer combined with blood smear was compared. The routine blood test results of the microscopic examination method and the simple blood cell analyzer test method. Results 1.Among the 84 blood samples, 40 were detected by blood cell analyzer, the positive detection rate was 47.6%(40/84), and the combined test reported 55 routine blood abnormalities, and the positive detection rate was 65.5%(55/84), after comparison, the positive detection rate of the two blood routine test methods was statistically significant(χ^2=4.748, P =0.029 <0.05);2. blood samples analysis of 84 samples showed that 38 cases, 24 cases, 30 cases, 18 cases, 2 cases and 4 cases of WBC, PLT, NE, LY,EO and MO. The positive rates were 45.2%, 28.6% and 35.7%, 21.4%, 2.4%, and 4.8%;combined test results showed that there were 40 cases, 26 cases, 33 cases, 19 cases, 10 cases and 13 cases, respectively, the positive rates were47.6%, 30.9%, 39.3%, 22.6%, 11.9%, and 15.5%. The positive detection rates of EO and MO in the two groups were statistically significant(EO: χ^2= 4.397, P=0.036;MO: χ^2=4.188, P=0.041 <0.05), the difference of other items was not statistically significant(P>0.05). Conclusion Combined blood cell analysis technique and blood smear cell morphology test technique in blood routine test can help to improve the detection rate of abnormal blood samples and make up for the deficiency of blood cell technology in cell morphology test.
