健康管理对老年耳鸣患者康复效果的研究

Study on the Effect of Health Management on the Rehabilitation of Elderly Patients with Tinnitus

目的研究健康管理对于老年慢性耳鸣患者预后的短期效果。方法选取2017年8月-2018年2月间在山西某三甲医院耳鼻喉头颈外科门诊就诊的150例老年慢性耳鸣患者作为研究对象,随机分组。对照组耳鸣患者75例,给予常规护理;干预组耳鸣患者75例,在常规护理的基础上健康管理的相应干预措施。在干预前后,采用耳鸣残疾评估量表(tinnitus handicap inventory, THI)、焦虑自评量表(self-rating anxiety scale, SAS)及抑郁自评量表(self-rating depression scale, SDS)对两组患者的耳鸣、焦虑、抑郁严重程度进行评估。结果干预后,干预组THI总分(25.6±16.84)分、SAS得分(49.58±3.56)分、SDS得分(34.25±5.21)分均低于对照组得分,差异有统计学意义(t=4.822、3.964、5.831,P<0.05)。就有效率而言,干预组有效率96.00%远高于对照组的有效率74.67%(χ^2=105.64,P=0.003)。结论健康管理可以缓解老年慢性耳鸣患者的耳鸣困扰,减少负面情绪的影响,可以在临床耳鸣治疗中推广。

Objective To study the short-term effects of health management on the prognosis of elderly patients with chronic tinnitus. Methods A total of 150 elderly patients with chronic tinnitus who were admitted to the otolaryngology head and neck surgery clinic of a top three hospital in Shanxi from August 2017 to February 2018 were randomly divided into the control group, 75 patients with tinnitus were given routine care;75 patients with tinnitus in the intervention group were treated with appropriate interventions for health management based on routine care. Before and after intervention, the tinnitus handicap inventory(THI), self-rating anxiety scale(SAS) and self-rating depression scale(SDS)were used to treat the two groups. The patient’s tinnitus, anxiety, and severity of depression were assessed. Results After intervention, the THI total score(25.6±16.84)points, SAS score(49.58±3.56)points,, and SDS score(34.25±5.21)points, were lower than the control group. In terms of efficiency.The differents was statistically significant(t =4.822,3.964,5.831,P<0.05). the intervention group of efficiency of 96.00% was much higher than the control group with an effective rate of 74.67% (χ^2=105.64,P =0.003). Conclusion Health management can alleviate the tinnitus troubles of elderly patients with chronic tinnitus and reduce the negative emotions, which can be promoted in the treatment of clinical tinnitus.
