银屑病临床指南国际制定现状分析

Analysis on the International Situation of the Development of Psoriasis Clinical Guidelines

目的针对国际银屑病临床指南的现状进行调查分析,为开展银屑病临床指南制修订的研究者及指南的应用者提供参考。方法检索2018年1月以前国际已经公开发布的银屑病临床指南,制定纳入标准、排除标准,分析指南发布年份、制定方法、发布的国家/地区、涉及的疾病类型、主要关注的临床问题和目标人群等。结果国际共发布英文及中文银屑病临床指南105篇,其中英文为95篇,循证指南为59篇,共识指南46部,循证指南发布呈现逐年增多的趋势,银屑病各疾病类型及涉及临床诊疗的各个环节均有相关指南发布。结论关节型银屑病、中重度银屑病的诊疗及银屑病生物制剂使用是目前临床指南关注的重点。脓疱病型银屑病、红皮型银屑病及特殊部位的银屑病临床指南、专门针对银屑病患者心理社会问题诊疗的指南较少,可作为未来指南制定的方向。银屑病临床指南数量较多、内容全面,临床中应用可优先选择循证指南,并综合制定时间、发表机构及指南质量等进行选择。

Objective To investigate and analyze the current situation of psoriasis clinical guidelines,and provide reference for the development direction and the application of psoriasis guidelines.Methods To retrieve the published psoriasis guidelines in the world before January 2018.Situation analysis of release year,developing method,developing country and region,types of diseases,major clinical problems and target population.Results A total of 105 psoriasis clinical guidelines in English and Chinese were included.There were 95 English articles,59 evidence-based guidelines,46 consensus guidelines,and the trend of evidence-based guidelines is increasing.The published guidelines are about clinical diagnosis and treatment of all types of psoriasis.Conclusion The focus of the current psoriasis guidelines is the diagnosis and treatment of psoriatic arthritis,moderate-to-severe plaque psoriasis and the use of biologics.The directions of future guidelines are pustular psoriasis,erythrodemic psoriasis,psoriasis at specific sites,and the diagnosis and treatment of psychosocial problems in patients with psoriasis.
