术前治疗性抚触联合中医护理干预对乳腺癌患者围手术期应激反应的影响

Effects of Preoperative Therapeutic Caress Combined with TCM Nursing Intervention on Perioperative Stress Response in Patients with Mammary Cancer

目的:探讨术前治疗性抚触联合中医护理干预对乳腺癌患者围手术期应激反应的影响。方法:将乳腺癌患者60例按入院先后顺序分为对照组与干预组各30例,2组均在术前接受常规护理和抚触护理,干预组在此基础上给予中医护理干预。比较2组患者入院时及术前的焦虑症状,术前1天以及手术5分钟、术毕5分钟的心率(HR)、平均动脉压(MAP)、血糖、皮质醇(Cor)水平、肾上腺素(E)及去甲肾上腺素(NE)水平。结果:干预组患者术前焦虑症状低于对照组(P<0.05);干预组患者术中、术后血糖水平和Cor水平均低于对照组(P<0.05),HR、MAP水平及E、NE水平与对照组比较无明显差异(P>0.05)。结论:术前治疗性抚触联合中医护理干预对减轻乳腺癌患者围手术期应激反应有一定作用,尤其是心理应激反应。

Objective: To explore the effects of preoperative therapeutic caress joined with TCM nursing inter vention on stress response of the patients with mammary cancer during perioperative period. Methods: Sixty patients were allocated to the control group and the intervention group according to visiting order, 30 cases each group, both groups accepted conventional nursing and caress nursing before the surgery, the intervention group was given TCM nursing intervention based on the therapy the control group accepted. To compare anxiety symptoms of the two groups when admitted and before the surgery, heart rate(HR), mean arterial pressure(MAP), the levels of blood glucose, cortisol(Cor), epinephrine(E) and norepinephrine(NE) one day before surgery, five minutes during the surgery and after the surgery. Results: Anxiety of the intervention group before the surgery was lower than that of the control group(P<0.05);the intervention group was lower than the control group in the levels of blood glucose and Cor during and after the surgery(P<0.05), there was no obvious difference in HR, the levels of MAP, E and NE compared with the control group(P>0.05). Conclusion: Preoperative therapeutic caress joined with TCM nursing intervention show present certain effects on stress of the patients with mammary cancer during perioperative period,especially psychological stress.
