腹泻型肠易激综合征伴焦虑抑郁状态的中西医诊治进展

Integrative Medicine Diagnostic and Therapeutic Progress of IBS-D Complicated with Anxiety and Depression Status

从西医对腹泻型肠易激综合征(diarrhea type irritable bowel syndrome,IBS-D)伴焦虑抑郁状态诊治现状、中医对IBS-D伴焦虑抑郁状态诊治现状2方面入手探讨IBS-D伴焦虑抑郁状态的中西医诊治进展,指出IBS-D合并焦虑抑郁状态的诊治目前较为困难,西医应用抗焦虑抑郁药物治疗本病时有适应症、且存在服用疗程、停药指征不明确、患者缺乏正确认知等问题;中医基于整体思想理论,辨证施治,患者接受程度好,中草药治疗本病疗效可靠。但由于中医药治疗无固定分型标准,样本量小、疗程不一等,在现有国内外疗效评价体系的基础上进一步建立具有传统中医药特色的临床疗效评价体系,能促进中医药治疗IBS-D伴焦虑抑郁状态的临床评价体系向规范化方向发展;在多学科协助诊治的大趋势下,发挥中西医结合治疗模式,在IBS-D伴焦虑抑郁状态的诊治方面发挥新的优势。

Integrative medicine diagnostic and therapeutic progress of diarrhea type irritable bowel syndrome(IBS-D) complicated with anxiety and depression status are discussed from diagnostic and therapeutic status in Western medicine, and TCM diagnostic and therapeutic status, it’s pointed out that the diagnosis and treatment for IBS-D complicated with anxiety and depression status are difficult in clinic currently, the problems such as indication, the course, undifined signs of drug withdrawal, lack of right cognition exist when anti-anxiety and depression herbs are used in Western medicine therapy for the disease;while patients could accept TCM therapy better, materia medica could obtain reliable clinical effects in the treatment for the disease based on the concept of holism, syndrome differentiation and treatment, TCM-featured evaluation system for clinical effects needs to be established on the foundation of current clinical effects evaluation system at home and abroad since the problems including no definite classification standard, small sample size, different course and others exist, the establishment of the system could promote standardized development of clinical evaluation system for TCM in the treatment for IBS-D complicated with anxiety and depression status, integrative medicine could exploit the advantages in the diagnosis and treatment for IBS-D complicated with anxiety and depression status.
