四维超声检查在胎儿畸形产前诊断中的应用价值 被引量:3

摘要 目的探讨四维超声检查在胎儿畸形产前诊断中的应用价值。方法回顾性分析于我院进行产前检查的1 500例孕产妇的临床资料,所有孕产妇产前均行二维超声筛查,同时再行四维超声检查。以胎儿娩出后的临床诊断作为金标准,比较两种检测方法的诊断效能。结果 1 500例孕产妇共娩出胎儿1 528例,经临床诊断畸形胎儿为22例,心脏、肺部、面部、神经系统畸形、颈部水囊瘤、骨折分别为2、3、5、8、2、2例。四维超声检查诊断的阳性预测值和灵敏度均高于二维超声检查(P<0.05)。结论四维超声检查诊断产前胎儿畸形灵敏度高,可以提高阳性预测值,相较于二维超声检查具有一定的优越性,是更为有效的产前胎儿畸形检查的诊断手段。 Objective To explore the application value of four-dimensional ultrasound in prenatal diagnosis of fetal malformation. Methods The clinical data of 1 500 maternals who underwent prenatal examination in our hospital were retrospectively analyzed. All the maternals were screened by two-dimensional ultrasound before delivery, and then examined by four-dimensional ultrasound. With the clinical diagnosis after delivery as the gold standard, the diagnostic efficiency of the two detection methods was compared. Results There were 1 528 fetuses delivered by 1 500 maternals, and22 cases were clinically diagnosed as malformed fetuses. The malformations of heart, lung, face, nervous system, neck hygroma and fracture were 2, 3, 5, 8, 2 and 2 cases respectively. The positive predictive value and sensitivity of four-dimensional ultrasound examination were higher than those of two-dimensional ultrasound examination(P <0.05).Conclusion Four-dimensional ultrasound examination has a high sensitivity in the diagnosis of prenatal fetal malformations, which can improve the positive predictive value. Compared with two-dimensional ultrasound examination, it has certain advantages and is a more effective diagnostic method for prenatal fetal malformations.
