EN制剂支持结合吞咽功能导向性训练对脑卒中并吞咽障碍患者的治疗效果 被引量:2

摘要 目的观察短肽型肠内营养(EN)制剂支持结合吞咽功能导向性训练对脑卒中并吞咽障碍患者的治疗效果。方法根据随机双盲法将2018年1月至2019年2月于我院就诊的90例脑卒中并吞咽障碍患者分为对照组和研究组,每组45例。两组均接受内科常规治疗,对照组在此基础上实施常规营养支持及常规吞咽功能训练,研究组实施EN制剂支持结合吞咽功能导向性训练。比较两组患者治疗效果。结果治疗后,两组前清蛋白、清蛋白、血红蛋白水平均显著升高,且研究组高于对照组(P<0.05);治疗后,两组洼田饮水试验评分、渗透/误吸量表评分均降低,且研究组低于对照组(P<0.05);治疗后,两组FOIS评分均升高,且研究组高于对照组(P<0.05);研究组不良反应总发生率显著低于对照组(P<0.05)。结论 EN制剂支持结合吞咽功能导向性训练治疗脑卒中并吞咽障碍可提高营养支持效果,改善患者的吞咽功能,且较为安全。 Objective To observe the clinical effect of short peptide enteral nutrition(EN) preparation support combined with task-oriented training on swallowing function in the treatment of patients with stroke and dysphagia. Methods A total of 90 stroke patients with dysphagia treated in our hospital from January 2018 to February 2019 were divided into control group and study group according to the randomized double-blind method, with 45 cases in each group. Both groups received routine medical treatment. The control group received routine nutritional support and swallowing function training,while the study group received EN preparation support combined with task-oriented training on swallowing function. The therapeutic effects were compared between the two groups. Results After treatment, the levels of prealbumin, albumin and hemoglobin in both groups increased significantly, and those in the study group were higher than the control group(P<0.05). After treatment, the scores of water swallow test and osmosis/aspiration scale in both groups decreased, and those in the study group were lower than the control group(P<0.05). After treatment, the FOIS scores of the two groups improved,and that in the study group was higher than the control group(P <0.05). The total incidence of adverse reactions in the study group was significantly lower than that in the control group(P<0.05). Conclusion EN preparation support combined with task-oriented training on swallowing function can improve effect of nutritional support and swallowing function of patients with stroke and dysphagia with higher safety.
