奥沙利铂联合多西他赛治疗晚期结肠癌的效果及对环氧化酶-2和前列腺素E2的影响

Effect of oxaliplatin combined with docetaxel on advanced colon cancer and on serum cyclooxygenase-2 and prostaglandin E2

目的分析奥沙利铂联合多西他赛治疗晚期结肠癌的效果及对血清环氧化酶-2(COX-2)和前列腺素E 2(PGE 2)的影响,为临床治疗提供参考。方法以晚期结肠癌患者为研究对象,将200例患者按治疗方法均分为2组:奥沙利铂治疗(对照组)和奥沙利铂联合多西他赛治疗(观察组)。分析2组患者的临床效果及治疗前后血清中COX-2、PGE 2、基质金属蛋白酶(MMPs)和基质金属蛋白酶抑制剂(TIMPs)的水平变化。结果与对照组比较,观察组患者的总有效率明显升高,且2组间差异存在显著的统计学意义(χ^2=3.554,P<0.05);对照组和观察组患者治疗后血清中COX-2、缺氧诱导因子-α(HIF-α)、增殖细胞核抗原(PCNA)、PGE 2、血管内皮生长因子(VEGF)、血管内皮生长因子受体(VEGFR)、基质金属蛋白酶(MMP-2,MMP-9)及金属蛋白酶抑制剂(TIMP-1,TIMP-2)水平均明显降低(P<0.05),且观察组患者的上述因子降低更明显(P<0.05)。结论奥沙利铂联合多西他赛对晚期结肠癌疗效较好,能有效降低患者血清中COX-2和PGE 2水平。

Objective To analyze the efficacy of oxaliplatin combined with docetaxel in the treatment of advanced colon cancer and on serum cyclooxygenase 2(COX-2)and prostaglandin E 2(PGE 2),provide reference for clinical treatment.Methods The patients with advanced colon cancer diagnosed and treated were selected as the research subjects.200 patients were divided into 2 groups according to the treatment method:oxaliplatin treatment(control group)and oxaliplatin combined with docetaxel treatment(observation group).The clinical effects of the 2 groups of patients and the changes of serum COX-2,PGE 2,MMPs and TIMPs levels before and after treatment were analyzed.Results Compared with the control group,the total effective rate of the observation group was significantly higher,and the difference between the 2 groups was statistically significant(χ^2=3.554,P<0.05).The serum levels of COX-2,HIF-α,PCNA,PGE 2,VEGF,VEGFR,MMPs(MMP-2,MMP-9)and TIMPs(TIMP-1,TIMP-2)in the control group and the observation group were significantly lower than those before treatment(P<0.05).The above factors in the observation group decreased more significantly(P<0.05).Conclusion Oxaliplatin combined with docetaxel has a better effect on advanced colon cancer,and can effectively reduce the serum levels of COX-2 and PGE 2.
