膀胱癌患者血清血小板因子4的表达及临床意义

Expression of serum platelet factor 4 in patients with bladder cancer and its clinical significance

目的探讨血清血小板因子4(PF4)与膀胱癌、血清缺氧诱导因子-1α(HIF-1α)及血管内皮生长因子(VEGF)水平的关系。方法选取2014年1月—2015年12月宁波市医疗中心李惠利医院泌尿外科收治的膀胱癌患者130例作为膀胱癌组,选取同期该院健康体检者130例作为对照组。测定血清PF4及HIF-1α、VEGF水平。结果膀胱癌组血清PF4、HIF-1α及VEGF水平均高于对照组(P<0.05)。肿瘤分期T2~T4患者血清PF4水平高于Ta、T1(P<0.05)。单因素分析结果显示,肿瘤大小、肿瘤分期、淋巴结转移、PF4、HIF-1α及VEGF水平与膀胱癌的预后有关(P<0.05);多因素分析结果显示,肿瘤分期、淋巴结转移、PF4、HIF-1α及VEGF水平为膀胱癌预后的独立危险因素(P<0.05)。膀胱癌患者血清PF4与血清HIF-1α、VEGF水平均呈正相关(r=0.591、0.624,P<0.05)。结论膀胱癌患者血清PF4水平升高,血清PF4水平与膀胱癌分期、预后及其新生血管生成关系密切。

Objective To investigate the relationship between serum platelet factor 4(PF4)and bladder cancer,the serum levels of hypoxia-inducible factor-1α(HIF-1α)and vascular endothelial growth factor(VEGF).Methods Selected 130 patients with bladder cancer were enrolled as bladder cancer group(group B)and 130 patients with healthy physical examination were enrolled as control group(group C)from January 2014 to December 2015 in the Department of Urology of Ningbo Medical Center Lihuili Eastern Hospital.The serum PF4 and neovascularization index(HIF-1α,VEGF)levels were determined.Results The serum levels of PF4,HIF-1αand VEGF in group B were higher than those in group C(P<0.05).The serum PF4 levels of pathological grade T2-T4 patients were higher than Ta-T1(P<0.05).Univariate results showed that tumor size,tumor stage,lymph node metastasis and serum PF4,HIF-1α,VEGF levels were associated with the prognosis of bladder cancer(P<0.05).Multivariate analysis showed that tumor stage,lymph node metastasis and serum PF4,HIF-1αand VEGF levels were independent risk factors for bladder cancer prognosis(P<0.05).Serum PF4 was positively correlated with serum HIF-1αand VEGF levels in patients with bladder cancer(r=0.591and 0.624,P<0.05).Conclusion Serum PF4 levels are elevated in patients with bladder cancer,and serum PF4 levels are closely related to bladder cancer stage,prognosis and neovascularization.
