Lamaze呼吸法联合自由体位分娩对初产妇分娩疼痛及妊娠结局的影响

Effect of lamaze breath method combined with free posture delivery on labor pain and pregnancy outcomes of primipara

目的探讨Lamaze呼吸法联合自由体位分娩对产妇分娩疼痛及妊娠结局的影响。方法选取2014年12月~2017年12月在我院就诊的初产妇300例,按照分娩方式不同分为观察组和对照组,每组150例。观察组孕妇接受Lamaze呼吸法减痛分娩训练,同时产时采用自由体位分娩;对照组孕妇不进行Lamaze呼吸法减痛分娩训练联合自由体位分娩;比较分析两组孕妇分娩疼痛程度、产程进展情况、分娩方式、Apgar评分、产后2小时出血量及患者满意度。结果与对照组比较,观察组孕妇分娩3级疼痛程度、各产程时间、总产程时间、产后2小时出血量均明显低于对照组,差异有统计学意义(χ2=61.364,t=-10.681、-36.072、-9.730、-16.073、-24.582,P均为0.000);且观察组初产妇顺产分娩率高于对照组,而中转剖宫产率、阴道助产率低于对照组,差异有统计学意义(χ2=5.882、5.882、4.704,P均<0.05);观察组新生儿Apgar评分及患者满意度均高于对照组(t=5.223,P=0.000;χ2=12.275,P=0.000),而总不良反应发生率明显低于对照组,差异有统计学意义(χ2=3.939,P=0.047)。结论Lamaze呼吸法联合自由体位分娩可以有效减轻孕妇分娩疼痛,缩短产程,提高孕妇自然分娩率,减少阴道助产及剖宫产,减少产后出血及增加患者满意度方面均有积极推动作用。

Objective To explore the effect of Lamaze breath method combined with free posture delivery on labor pain and pregnancy outcomes of primipara.Methods 300 primipara who were treated in our hospital from December 2014 to December 2017 were selected.According to the delivery mode,they were divided into the observation group and the control group,with 150 cases in each group.Pregnant women in the observation group were given Lamaze breath pain-reduction delivery training and they adopted free posture in delivery.Pregnant women in the control group were not given Lamaze breath pain-reduction delivery training combined with free posture delivery.Labor pain degree,labor progress situation,labor mode,Apgar scores,blood loss during 2 hours after labor and patients’satisfaction of the two groups were compared and analyzed.Results Compared with the control group,the degree of grade 3 labor pain,duration of different stages of labor,the total time of labor and blood loss during 2 hours after labor in the observation group were significantly lower than those of the control group.The differences were statistically significant(χ2=61.364,t=-10.681,-36.072,-9.730,-16.073,-24.582,all P=0.000).The rate of spontaneous delivery in the observation group was higher than that in the control group while the rates of transferring to cesarean section and vaginal delivery were lower than those in the control group(χ2=5.882,5.882,4.704,all P<0.05).Apgar scores of newborns and satisfaction of patients of the observation group were both higher than those of control group(t=5.223,P=0.000;χ2=12.275,P=0.000),while the total adverse reaction rate was significantly lower than that of control group(χ2=3.939,P=0.047).Conclusion Lamaze breath method combined with free posture delivery has positively promoting significance in effectively reducing labor pain,shortening labor,improving natural delivery of pregnant women,reducing vaginal assistant labor and cesarean section,decreasing postpartum hemorrhage and improving the satisfaction degree of patients.
