论太阴病“自利”成因及其治法

Etiology of Spontaneous Diarrhea and its Treatment Strategy in Taiyin Disease

《伤寒论》277条记载“自利不渴者,属太阴”,太阴病“自利”是太阴湿土之气犯脾所致,“脾胃为后天之本”,脾阳是运化水谷等的动力,在太阴病初起阶段,患者脾阳受损不重,湿多寒少,属于湿邪自病,故治湿为其正治。本文围绕着“自利”这一病,通过“太阴自利”与“少阴自利”的区别;太阴病“自利”的发病机理;太阴病“自利”的治法;太阴病“自利”的禁治法4个方面的讨论,对太阴病“自利”进行六气解析。同时根据《内经》六气治法,探讨太阴“自利”的治法并根据气味推荐常规用药,以期对临床有一定的借鉴和参考。

In Treatise on Cold Damage,clause 277 records that patients present with spontaneous diarrhea but without thirsty,it pertains to Taiyin disease.The spontaneous diarrhea in Taiyin disease is caused by damp earth qi of Taiyin invading the spleen.The spleen and stomach is the root.The spleen yang promotes the transportation and transformation.In the initial stage of Taiyin disease,if the damage to patient′s spleen yang is mild,that is,more dampness less cold,which pathogenic dampness is in disorder.To treat pathogenic dampness is the right strategy.Emphasizing on the spontaneous diarrhea,this article want to discuss in the following four aspects:difference between diarrhea in Taiyin and in Shaoyin,pathogenesis of diarrhea in Taiyin disease,treatment strategy of diarrhea in Taiyin disease,and contraindication of diarrhea in Taiyin disease,and we want to analyze six qi on the spontaneous diarrhea of Taiyin disease.At the same time,according to the six qi treatment strategy in the Yellow Emperor′s Canon of Medicine,we explored the treatment principle of Taiyin spontaneous diarrhea and recommended common herbs based on their qi and flavor,which may provide some clinical reference.
