胸腔镜下行食管癌根治术的临床效果研究

Research on clinical curative effect of thoracoscopic radical resection of esophageal cancer

目的探讨胸腔镜下食管癌根治术的应用效果。方法选取2014年3月~2018年1月我院肿瘤科接诊的78例食管癌患者,采用随机数字表法将其分为两组,各39例。对照组实施传统食管癌根治术,研究组实施胸腔镜下食管癌根治术,比较治疗效果、安全性及术后疼痛程度。结果研究组手术时间短于对照组,术中出血量及术后引流量均少于对照组,下地时间及出院时间均早于对照组,并发症发生率低于对照组,淋巴结清扫数目多于对照组,术后不同时间段疼痛评分均低于对照组,差异有统计学意义(P<0.05)。术后随访1年结果显示,观察组疾病复发率为2.56%,明显低于对照组15.79%,差异有统计学意义(P<0.05)。结论食管癌患者实施胸腔镜下食管癌根治术效果佳、安全性高且疼痛感轻,值得推广应用。

Objective To explore the application effect of thoracoscopic radical resection of esophageal cancer.Methods 78 patients with esophageal cancer who were treated in department of oncology in our hospital from March 2014 to January 2018 were selected.According to the random number table,they were divided into two groups,with 39 cases in each group.The control group was given traditional radical resection of esophageal cancer while the study group was given thoracoscopic radical resection of esophageal cancer.The curative effect,safety and postoperative pain degree were compared.Results The operation time of the study group was shorter than that of the control group.The intraoperative blood loss and postoperative drainage volume were lower than those of the control group.The leaving bed time and discharge time were earlier than the control group.The complication rate was lower than that of the control group and the number of lymph node dissection was more than that of the control group.The postoperative pain scores at different time points of the study group were lower than those in the control group.The differences were significant(P<0.05).The results of one-year follow-up showed that the recurrence rate of the observation group was 2.56%,which was significantly lower than that of the control group(15.79%).The difference was statistically significant(P<0.05).Conclusion Patients with esophageal cancer undergoing thoracoscopic radical resection of esophageal cancer have good curative effect,high safety and light pain.It is worthy of promotion and application.
