髋关节镜手术治疗髋臼盂唇钙化沉积病

Clinical Outcomes of Hip Arthroscopic Treatment for Acetabular Labrum Calcification Deposition

目的探讨髋关节镜手术治疗髋臼盂唇钙化沉积病的临床疗效。方法2012年12月~2018年8月行髋关节镜手术治疗髋臼盂唇钙化沉积病12例,采用疼痛视觉模拟评分(Visual Analogue Scale,VAS)、改良髋关节Harris评分(modified Harris Hip Score,mHHS)及髋关节非骨关节炎评分(Nonarthritic Hip Score,NAHS)进行髋关节功能评估。结果手术均顺利完成,术后随访(16.2±8.8)月(12~36个月)。末次随访时VAS由术前的(4.2±1.7)分降至(1.5±0.9)(t=6.490,P=0.000);mHHS由(53.6±12.6)分提高至(80.3±8.9)分(t=7.853,P=0.000);NAHS评分由(57.0±12.5)分提高至(79.0±9.5)分(t=5.338,P=0.000)。无血管、神经损伤,无伤口感染、股骨头坏死、异位骨化等并发症,无需翻修、髋关节置换。结论髋关节镜手术治疗髋臼盂唇钙化沉积病能显著缓解疼痛,改善髋关节功能,是一种安全有效的治疗方式。

Objective To investigate clinical outcomes of hip arthroscopic treatment for acetabular labrum calcification deposition.Methods A total of 12 patients with acetabular labrum calcification deposition were treated by hip arthroscopic procedure from December 2012 to August 2018.The hip function was evaluated by Visual Analogue Scale(VAS),modified Harris Hip Score(mHHS)and Nonarthritic Hip Score(NAHS).Results All the operations were successful.The mean follow-up period was(16.2±8.8)months(range,12-36 months).At the last follow-up,the mean VAS scores were improved from(4.2±1.7)points preoperatively to(1.5±0.9)points(t=6.490,P=0.000),the mHHS scores were improved from(53.6±12.6)points to(80.3±8.9)points(t=7.853,P=0.000),and the NAHS scores from(57.0±12.5)points to(79.0±9.5)points(t=5.338,P=0.000).During the follow-up period,there were no vascular or nerve injury,wound infection,femoral head necrosis,heterotopic ossification or other complications,and no cases of revision or hip replacement were noted.Conclusions Hip arthroscopic treatment for acetabular labrum calcification deposition has advantages of minimal invasion,less complications and quick recovery,significantly relieving pain and improving the function of hip joint.It is a safe and effective treatment method for this kind of disease.
