前列地尔联合骨化三醇治疗慢性肾衰竭的疗效及对血清ANG Ⅱ、MCP-1、CTGF的影响 被引量:10

摘要 目的 研究前列地尔联合骨化三醇治疗慢性肾衰竭的疗效及对血清血管紧张素Ⅱ(angiotensin,ANGⅡ)、人单核 细胞趋化蛋白-1(monocyte chemoattractant protein-1,MCP-1)、结缔组织生长因子(connective tissue growth factor,CTGF)的 影响。方法 选择2014年6月 2018年6月在湖北省钟祥市人民医院接受治疗的102例慢性肾衰竭患者,按照随机数字表法分为 观察组和对照组,对照组采用前列地尔治疗,观察组以对照组为基础联合骨化三醇治疗。观察2组临床疗效,分析比较治疗前后 2组血清ANGⅡ、MCP-1、CTGF水平变化以及肾功能指标水平。结果 治疗后,观察组总有疗效率明显高于对照组总有效率(P <0.05);治疗后,2组血清ANGⅡ、MCP-1、CTGF水平均明显降低,且观察组显著低于对照组,差异有统计学意义(P<0.05);治疗后,2组患者肾功能均得到有效改善,且观察组血清肌酐清除率(creatinine clearance rate,Ccr)水平显著高于对照组,肌酐 (creatinine clearance,Cr)、尿素氮(BUN)、尿酸均显著低于对照组,差异具有统计学意义(P<0.05)。结论 使用前列地尔 与骨化三醇联合治疗慢性肾衰竭可有效改善患者肾功能,提高其临床疗效,同时减低患者血清MCP-1、ANGⅡ、CTGF水平。 Objective To study the efficacy of alprostadil combined with calcitriol in the treatment of chronic renal failure and its effects on serum ANGⅡ, MCP-1, CTGF. Methods A total of 102 patients with chronic renal failure who had received therapy in our hospital between June 2014 and June 2018 were selected as the subjects. According to the random number table method, they were divided into the observation group and control group. The control group was treated with alprostadil, while the observation group was treated with alprostadil combined with calcitriol. The curative effect of the two groups was observed, while levels of serum plasma angiotensinⅡ(ANG Ⅱ), monocyte chemotactic protein 1(MCP 1), connective tissue growth factor(CTGF)and renal indexes were compared between the two groups before and after treatment. Results After treatment, the total therapeutic effect of the observation group was higher than that of the control group(P<0.05). Serum ANGⅡ, MCP-1, CTGF levels were reduced in both groups, especially in the observation group(P<0.05). After treatment, renal function was effectively improved in both groups. The serum Ccr level of the observation group was higher than that of the control group, but serum levels of Cr, BUN and uric acid were lower, with statistically significant difference(P<0.05). Conclusion Alprostadil combined with calcitriol used in the treatment of chronic renal failure can effectively improve the renal function, enhance the clinical effect, and reduce patients' serum MCP - 1, ANG Ⅱ, CTGF levels.
