目的探索并构建乳腺癌并发抑郁患者的阶梯式护理模式。方法通过德尔菲法对19名相关领域专家进行2轮函询确定腺癌并发抑郁患者的阶梯式护理模式。该模式包括42个条目,涵盖患者抑郁筛查、评估、干预、随访等内容。函询专家包括乳腺癌临床和护理专家、护理管理专家、心理咨询师、精神病学专家。应用Likert-5分度量法对条目重要性进行打分。最终形成乳腺癌并发抑郁患者的阶梯式护理模式及流程图。结果函询专家对模式中的42项条目意见趋同,达成专家共识。乳腺癌并发抑郁患者的阶梯式护理模式包括乳腺癌患者抑郁的初筛/复筛、评估、阶梯式护理干预、随访复查、转介等内容。结论本研究基于文献查阅及专家函询构建的乳腺癌并发抑郁患者的阶梯式护理模式的构建过程科学、合理,值得在临床推广应用。

Objective To study a stepped-care model for breast cancer patient with depression. Methods Through Delphi method, 19 experts in related fields were consulted in two rounds to determine the nursing mode. The model includes 42 items including depression screening, evaluation, intervention, follow-up, etc. The consultation experts include breast cancer medical and nursing experts, nursing management experts, psychological counselors and psychiatric experts. Likert 5-scale method was applied to score the item importance, The identified model is shown in the form of flow charts. Results The experts reached agreement on 42 items. Stepped care mode for patients with breast cancer complicated with depression includes screening, evaluation, stepped care intervention, follow-up review, referral, etc. Conclusions This study is based on literature review and expert consultation to build up a stepped care model for patients with breast cancer complicated with depression. The model development process is scientific and reasonable, which is worthy to be popularized and applied in clinic.
