磷脂络合碘联合羟苯磺酸钙胶囊治疗糖尿病高血压视网膜病变的效果

Effect of Phospholipid Complex Iodine combined with Calcium Dobesilate Capsules in the treatment of diabetic hypertensive retinopathy

目的探讨磷脂络合碘联合羟苯磺酸钙胶囊治疗糖尿病高血压视网膜病变的效果。方法选取2017年1月~2018年12月我院收治的80例(142眼)糖尿病高血压视网膜病变患者作为研究对象,按照随机数字表法将其分为对照组和观察组,每组各40例(71眼)。对照组患者采用磷脂络合碘治疗,观察组患者采用羟苯磺酸钙胶囊联合磷脂络合碘治疗。比较两组患者的视力改善情况、黄斑囊性水肿吸收率及治疗前后空腹血糖、血清果糖胺、平均动脉压、血管内皮素生长因子、一氧化氮、全血低切黏度、全血高切黏度、血浆黏度、血小板黏附率、血小板聚集率。结果观察组患者的视力改善情况明显优于对照组,差异有统计学意义(P<0.05)。观察组患者的黄斑囊性水肿吸收率高于对照组,差异有统计学意义(P<0.05)。两组患者治疗前的空腹血糖、血清果糖胺水平及平均动脉压比较,差异无统计学意义(P>0.05);两组患者治疗前后的空腹血糖、血清果糖胺水平比较,差异无统计学意义(P>0.05);两组患者治疗后的平均动脉压均低于治疗前,差异有统计学意义(P<0.05);两组患者治疗后的空腹血糖、血清果糖胺水平及平均动脉压比较,差异无统计学意义(P>0.05)。两组患者治疗前的血管内皮素生长因子及一氧化氮水平比较,差异无统计学意义(P>0.05);对照组患者治疗前后的血管内皮素生长因子及一氧化氮水平比较,差异无统计学意义(P>0.05);观察组患者治疗后的血管内皮素生长因子水平低于治疗前、对照组,一氧化氮水平高于治疗前、对照组,差异有统计学意义(P<0.05)。两组患者治疗前的全血低切黏度、全血高切黏度、血浆黏度、血小板黏附率以及血小板聚集率比较,差异无统计学意义(P>0.05);对照组患者治疗前后的全血低切黏度、全血高切黏度、血浆黏度比较,差异无统计学意义(P>0.05);对照组患者治疗后的血小板黏附率与血小板聚集率均低于治疗前,差异有统计学意义(P<0.05);观察组患者治疗后的全血低切黏度、全血高切黏度、血浆黏度、血小板黏附率以及血小板聚集率均低于治疗前、对照组,差异有统计学意义(P<0.05)。结论羟苯磺酸钙联合磷脂络合碘治疗糖尿病高血压视网膜病变的效果确切,可有效提升患者的视力水平,改善其眼部血液流变学指标。

Objective To investigate the effect of Phospholipid Complex Iodine combined with Calcium Dobesilate Capsules in the treatment of diabetic hypertensive retinopathy.Methods Eighty patients(142 eyes)with diabetic hypertensive retinopathy admitted to our hospital from January 2017 to December 2018 were enrolled in the study.They were divided into control group and observation group according to the random number table method,40 cases(71 eyes)in each group.The patients in the control group were treated with Phospholipid Complex Iodine,and the patients in the observation group were treated with Calcium Dobesilate Capsules combined with Phospholipid Complex Iodine.The improvement of visual acuity,absorption rate of macular cystic edema,fasting blood glucose,serum fructosamine,mean arterial pressure,vascular endothelin growth factor,nitric oxide,whole blood low-cut viscosity,whole blood high-viscosity,plasma viscosity,platelet adhesion rate,platelet aggregation rate before and after treatment were compared between the two groups.Results The improvement of visual acuity in the observation group was significantly better than that in the control group,and the difference was statistically significant(P<0.05).The absorption rate of macular cystic edema in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).There were no significant differences in fasting blood glucose, serum fructosamine levels and mean arterial pressure between the two groups before treatment (P>0.05). There were no significant differences in fasting blood glucose and serum fructosamine levels in the two groups between before treatment and after treatment (P>0.05). The mean arterial pressure after treatment in both groups was lower than that before treatment, and the difference was statistically significant (P<0.05). There were no significant differences in fasting blood glucose, serum fructosamine levels and mean arterial pressure between the two groups after treatment (P>0.05). There were no significant differences in the levels of vascular endothelin growth factor and nitric oxide between the two groups before treatment (P>0.05). There were no significant differences in the levels of vascular endothelin growth factor and nitric oxide in the control group between before treatment and after treatment (P>0.05). The level of vascular endothelin growth factor in the observation group after treatment was lower than that before treatment and in the control group, the level of nitric oxide was higher than that before treatment and in the control group, and the differences were statistically significant (P<0.05). There were no significant differences in the whole blood low-cut viscosity, whole blood high-viscosity, plasma viscosity, platelet adhesion rate and platelet aggregation rate between the two groups before treatment (P>0.05). There were no significant differences in the whole blood low-cut viscosity, whole blood high-viscosity and plasma viscosity of the control group between before treatment and after treatment (P>0.05). The platelet adhesion rate and platelet aggregation rate of the control group after treatment were lower than those before treatment, and the differences were statistically significant (P<0.05). The whole blood low-cut viscosity, whole blood high-viscosity, plasma viscosity, platelet adhesion rate and platelet aggregation rate in the observation group after treatment were lower than those before treatment and in the control group, and the differences were statistically significant (P<0.05). Conclusion Calcium Dobesilate Capsules combined with Phospholipid Complex Iodine is effective in the treatment of diabetic hypertensive retinopathy, which can effectively improve the visual acuity level and improve the blood rheology of patients.
