经鼻内镜鼻窦手术后鼻腔填塞膨胀海绵在慢性鼻炎及鼻窦炎患者中的应用效果

Application effect of nasal cavity filled with expanding sponge after endoscopic sinus surgery in patients with chronic rhinitis and sinusitis

目的分析经鼻内镜鼻窦手术(ESS)后鼻腔填塞膨胀海绵在慢性鼻炎及鼻窦炎患者中的应用效果。方法选取2014年1月~2018年6月我院采用ESS治疗的92例慢性鼻炎及鼻窦炎患者,根据其术后鼻腔是否填塞膨胀海绵分为观察组(49例,填塞膨胀海绵)和对照组(43例,未填塞膨胀海绵),比较两组的手术效果、术腔功能恢复情况及术后并发症总发生率。结果观察组的治愈率(77.6%)、总有效率(95.9%),显著高于对照组(53.5%、83.7%)(P<0.05);术后6个月观察组在鼻腔疼痛、鼻塞、流涕、味觉功能四项的鼻腔鼻窦结局测试22条(SNOT-22)分项评分值及SNOT-22总评分值均显著高于对照组(P<0.05);观察组术后6个月的并发症率总发生为8.2%(4/49),显著低于对照组的32.6%(14/43),差异有统计学意义(P<0.05)。结论ESS术后鼻腔填塞膨胀海绵,可促进术后鼻腔功能恢复,提升手术效果并有效降低相关并发症的发生,值得在临床推广。

Objective To analyze the application effect of nasal cavity filled with expanding sponge after endoscopic sinus surgery(ESS)in patients with chronic rhinitis and sinusitis.Methods A total of 92 cases of chronic rhinitis and sinusitis treated with ESS in our hospital from January 2014 to June 2018 were selected.According to whether nasal cavity filled with expanding sponge after operation the patients were divided into observation group(49 cases,nasal cavity was filled with expanding sponge)and control group(43 cases,no nasal cavity was filled with expanding sponge).The operative effect,functional recovery and total complications of the two groups were compared.Results The cure rate(77.6%)and total effective rate(95.9%)of the observation group were significantly higher than those of the control group(53.5%,83.7%)(P<0.05).The scores of 22 tests on the outcome of nasal cavity and sinuses(SNOT-22)and total score of SNOT-22 in nasal pain,nasal obstruction,runny nose and taste function of the observation group were significantly higher than those of the control group in 6 months after operation(P<0.05).The total complication rate in 6 months after operation was 8.2%(4/49),which was significantly lower than that in the control group 32.6%(14/43),with statistical difference(P<0.05).Conclusion Nasal cavity filling with expanding sponge after ESS can promote the recovery of nasal function,improve the effect of operation and effectively reduce the related complications.It is worth popularizing in clinical practice.
