碘伏加湿润烧伤膏在鼻咽癌放射性皮肤反应中的应用及护理

Application of iodophor plus moisture exposed burn ointment in treatment of skin toxicity induced by radiotherapy in patients with nasopharyngeal carcinoma

目的总结碘伏加湿润烧伤膏在鼻咽癌患者Ⅲ级放射性皮肤反应中的应用和护理体会。方法回顾性分析总结2015年1月至2019年3月江苏省肿瘤医院收治的32例放疗中出现Ⅲ级放射性皮肤反应的鼻咽癌患者的临床资料,皮肤损伤处使用碘伏加湿润烧伤膏换药处理,同时护理中加强健康宣教和心理干预,及时调整换药频次。结果32例患者放射性皮肤损伤处经用碘伏加湿润烧伤膏换药1~2次后,伤口处疼痛即明显减轻,1~5 d后伤口渗出减少、收敛干燥,7~12 d伤口愈合,其中仅4例患者中断放疗2~5 d,基本不影响放疗疗程的完成,患者满意度高。结论碘伏加湿润烧伤膏处理鼻咽癌放疗后Ⅲ级皮肤反应,可以有效控制感染,促进伤口愈合;护理中加强创面管理以及健康宣教和心理干预,能够助力缩短伤口愈合时间,提高患者满意度。

Objective To evaluate the curative effect of iodophor plus moisture exposed burn ointment(MEBO)on nasopharyngeal carcinoma patients with skin toxicity of gradeⅢinduced by radiotherapy.Methods The clinical data of 32 nasopharyngeal carcinoma patients with skin toxicity of gradeⅢinduced by radiotherapy in our hospital from Jan.2015 to Mar.2019 were analyzed retrospectively.The lesions were treated with iodophor plus MEBO with health education and psychological intervention being performed.The usage of drug was changed based on the situation.Results The pain of wound obviously alleviated after once to twice of dressing changing with iodophor plus MEBO.Exudation of wound reduced in the initial 1-5 days and wound completely healed in 7-12 days significantly.Only 4 patients receiving radiotherapy postponed 2-5 days.All the patients kept good satisfaction.Conclusions Iodophor plus MEBO is effective in the treatment of gradeⅢskin toxicity induced by radiotherapy for nasopharyngeal carcinoma patients especially for the controlling of infection and acceleration of wound healing.The strengthening of wound management,health education and psychological intervention in nursing can shorten recovery time,which improves satisfaction rate of patients.
